Colistin

We are starting a course of inhaled colistin and oral Zithromax in an attempt to quash a persistent Stenotrophomonas . We've already been though Cipro,Bactrim, Tobramycin and Tymentin IV, TOBI and Zithromax. Anyone have experience with Colistin (inhaled) or Stenotrophomonas to share? Thanks.

Hi !We did Bactrim IV (instead of oral because it is more effective) and IV timetin for 28 days and beat Sten Maltophilia. This bug is usually sensitive to Bactrim. Perhaps a course of IV bactrim with Pipercillin with inhaled Collistin will work. If the bug is sensitive to Tobramycin you can do TOBI instead of Collistin. Get the culture results and discuss your options with the doctor. If you want Sten Malt eradicated you need to get it done quickly. The longer Sten Malt hangs in there, the more entrenched it becomes. IV bactrim is every 12 hours but is a large IV and takes about 45 minutes. However, it works like a charm. We had to home mix (prepare ours) which was no fun at all.Don Pdon.patin@gte.net

Thanks Don. Unfortunately we've been up against this for about 3 years now and the latest culture shows resistance to Bactrim and not much action by Tobra,hence the new combo.
