HTS prior to Pulmozyme

There is probably an existing thread on this topic, but I'll ask anyway! <img src="i/expressions/face-icon-small-smile.gif" border="0"> DD is starting HTS and was advised to administer HTS prior to Pulmozyme with essentially no wait time in between. I understand that HTS can decrease the efficacy of the Pulmozyme which is why they advised not to give Pulmozyme first. However, administering Pulmozyme immediately following HTS still seems too close...am I wrong?<br><br>Just sent an email to her CF nurse regarding the volume of HTS to administer. We received 15mL vials...<br>

Katie, i remember hearing that a wait time is good. We usually do HTS first thing in the morning, eat breakfast, get ready for the day and then do pulmozyme.
I can't tell you if that wait time makes any difference for Ethan, but we do it anyway <img src="i/expressions/face-icon-small-smile.gif" border="0">

Hi Katie
You are correct in thinking there should be a break. The high salt content of HTS somewhat destroys the pulmuzyme, or so I have been told by two different centres. 30 mins is the minimum break i have been advised to take between the two. Hopefully your clinic can confirm their protocol on this.

Wow, I have not been told to do this. We do the HTS first which is immediately followed by the Pulmozyme. I will question my son's pulmonologist about this...

When we first started HTS, we did it immediately before the pulmozyme and then the vest. We did it this way for a long time; until my daughter had a bad exacerbation and I was trying to figure out what would stop her from occasionally wheezing. The doc and I went over her neb regime and we moved her Pulmozyme treatment to last neb of the day to be done after her VEST right before bed. Other Cf docs have differing views; not wanting to have the pulmozyme sit all night; but this is what works for us now.

Crazy how many inconsistencies there are! We were given 15mL vials of 3% HTS but not told how much to actually neb. I assumed 15mL was too much and contacted our center...was told that we were sent the wrong vials and to give DD only 1mL for nebbing. One mL seems like nothing?! Thoughts?

I hardly ever have any break in between. I can tell a difference when I do the pulmozyme and times when I don't so something must be working. At times if I am out of clean/sterile nebs I will even use the same empty neb. I think even with the trio eflow they tell you to just do them in the same neb right after each other. I'd be interested if anyone has a link to a study of some type. Sometimes these things are more theories that get started and get taken as fact.

kitomd21 - the 3% comes in 15ml vials. I don't know if you can get it any another way. Most neb the 7%, which you have to mix or use the pari hyper-sal. I think 3-5ml is typically what one nebs at a time.
