My two daughters with CF use the Acapella. They also have the Vest. We use the Acapella in the mornings before school because it's so much faster and more convenient than the Vest. In the evenings they use the Vest. They do their nebs prior to doing the Acapella. As far as nebs with the Vest, they do them at the same time. I think the Acapella works well as long as they do it correctly. That's the only problem with it. If you rush through and don't breathe right, it's not going to be beneficial (but it's that way with anything).

Jason is seven and he used to do the acc in the am and the vest in the pm - both with nebs. We have found that he is much more productive using the acc and can make the secretions move better. Now he does the acc in the am and pm.

My daughter uses both vest and acapella in the morning and at bedtime. She will do the vest first for 30 minutes then follow that with the Acapella. She says the vest breaks things up and the acapella helps move it out.

I use it as my main physiotherapy. I tried the vest but all it did was bounce me around with no result, so I tried this and found it worked a whole lot better. I do my pulmosyme and my albuterol beforehand, then the acapella, then my colistin (or Tobi, whichever you use). Then I do my nasal lavage and Flonase. I can usually get a lot of mucus up with the acapella unless I'm sick, in which case I use either a manual CPT or a high-vibration massager to shake stuff loose, and use the acapella afterwards to get it up.
