EXERCISE FOR LIFE ATHLETIC SCHOLARSHIP PROGRAM
Presented by the Boomer Esiason Foundation

The Boomer Esiason Foundation (BEF) has created a scholar-athlete award to assist cystic fibrosis (CF) high school senior athletes pursuing undergraduate degrees. BEF is awarding one $10,000 scholarship to a female and one $10,000 to a male.

Goal: To improve the quality of life and increase the lifespan of individuals with CF through the power of daily physical exercise (jogging). As therapeutic developments increase the lifespan of CF patients so does the need for further education in the area of exercise.

Background of scholarship: "Exercise is the single most important thing in my life that keeps me healthy. Jogging is a universal activity that everyone can partake in without the need for an expensive gym or pool membership," states Jerry Cahill who is a 50-year-old with CF. Jerry has been exercising all his life in spite of having CF.

Jerry states, "Doctors and CF centers talk about it, but we need to get everyone with CF doing it - your lungs are muscles and need to be worked to keep them clear and strong. Besides being compliant to medication and therapy routines, exercise should be a part of everyone's daily routine."

The Boomer Esiason Foundation is a leader in the area of academic scholarships and transplant grants for individuals with cystic fibrosis. BEF is now looking to be on the cutting edge again in the education of exercise.

Overview of the Scholar-Athlete Program

Scholarships will be awarded annually on the basis of demonstrated financial need, academic accomplishment and athletic ability in the area of running. BEF will grant one female and one male with a $10,000 grant. Grants are made directly to the academic institution to assist in covering the costs of tuition and fees.

Candidate Evaluation

A high school senior who has CF and demonstrates scholastic ability, athletic ability (exercising on a regular basis), character, leadership, service to the community, need for financial assistance, and daily compliance to CF therapy.

The student/athlete should be jogging on a regular basis and training for the 1.5 mile run. All candidates will be judged on time.

Applicants are required to provide ALL of the following documents in order to be considered:
- Application
- Completed EFL training log (print from web site)
- One-page essay on importance of exercise and compliance (single space)
- Recent photo
- Letter from physician (on letterhead) confirming CF diagnosis and therapy routine.
- Most recent W2 form verification for both parents
- High school transcript
- Letter of acceptance from college institution
- Signed waiver

Deadline: Friday, June 29th, 2007 (postmarked)
