Hi,
I have found Pillboxie extremely useful. You can input your medications and what time you take them. It also has an alert. I also use Wellnote - not for the medications portion as I use Pillboxie for those - but for the history. I can keep track of hospital visits, tests, family history, etc. I have a few others but these two seem to be the ones I refer most often.

You should check out techcf.org. We review apps for iPad/iPhone. There is a medical section, which contain some pretty good apps.
