Is bacteria only diagnosed in culture? Does the infectous disease dr have to diagnose it?
I finished my antbx AND steroids for a regular 'flare up' and I feel worse now than I did before.
Now I am starting to have night sweats.
I was wondering if this is indicitive. How is it diagnosed?

Hi,
 I have Mycobacterium Abcessus ..  In the past I had had night sweats, fevers at times...  There are two tests (that i know of, there may be more) that confirm MAC, one being (& your dr should do this) an AFB sputum culture , it is a special sputum collection that is sent out to a lab that deals w/ this.  The cup is taller than a reg sputum container & has a middle tube & a red ball at the bottom.
 The other test is a CT scan to see if there are any cavities caused by the MAC..  My reg. center is in Manhattan, but I go to Denver National Jewish in Colorado every six mos as they specialize in this..

Hey there!
I've had MAC before (which is a combo of 2-3 different mycobacteria organisms), but now my main cultprit is M. Abscessus. My CF doc was the one who diagnosed it via sputum culture. They diagnose it via AFB smear (where they smear your sputum and do a special stain that stains Acid Fast bacteria...which is what mycobateria are). This test only tells that there are acid fast bacteria, there's a further sputum culture, I think, to determine which bacteria you actually have. Again, my CF doc was the one who ordered all of these. When M. abscessus came up every time I was sick (and every time meds wouldn't work), my CF doc involved an infectious disease doc. Now, they are in cahoots together. Sometimes my cultures are done by the ID doc, others are done by my CF doc. It might depend on your doc or hospital with how involved an infectious disease doc will get, at least if you're inpatient.
For me, the main diagnosing factor of mycobacterium were constant fevers. I was always a little feverish when I got sick, but one time I got SUPER sick with constant fevers above 102*F. There were some 104*F fevers, too, and boy did I feel awful! I've had M. abscessus since then, and am always feverish, but not as bad as that initial time. The only reason I ever have night sweats is when fevers break. I had some fevers with MAC, but again, not as bad or as frequent. The MAC was able to be cleared up pretty quickly with a couple IV courses and long term oral abx.

Basically what Erin said. I actually dont remember fevers back when I got diagnosed... I think I just wasnt feeling well and pfts didnt really improve after a long iv course etc. I had no appetite though thats for sure. My ped cf doc actually was supposed to be having all her patients sputum's routinely tested for AFB, but she found out they weren't following her protocol about 6 months later when I was in the hospital, so they tested me then and it was discovered I had M. Chelonae, which in about a year or two transformed/adapted into M. Abscessus for me.

I have never had a special sputum cup though, they just write the orders for specific AFB.

Good Luck, it may be that you are just being affected by your bugs more b/c you are at a lower lung function (I saw your post about being "too healthy" for transplant... thats exactly what happened to me so I am in limbo where my QOL is sucky enough to not really be able to do anything, but my tests show I am too healthy... blah)

oops and Joni is correct that normally with a positive AFB culture they proceed with a CT scan to see if they can see the circular thingy-magics of how it grows or something... (haha like that doctor terminology?)

I didn't have any major symptoms to alert me that I had a Microbacterium. I was shocked when the told me. Since then I have had night sweats and pathetic lung function numbers. I had a CT Scan around the time of them diagnosing me with this new bug.

We are still struggling with my lung numbers. My last tune up my numbers when down rather than up. Really frustrating.
