No ovulation...

Hi,

So, I have been trying to time my cycles, figure out when ovulation is occurring so I can start my baby plan.

About 4 months ago, I had a good cycle where the monitor determined i had a peak fertility period. Since then, it's been downhill... 2 or 3 past cycles with low fertility index on the monitor, no change during the whole testing period. I also went 60 days between a cycle, requiring provera to induce a period.

So, this cycle, i tested for a LH surge. The OPKs never were dark like the control. I had cervical mucous day 16, and day 18, my OPK line was so light that I thought that maybe day 17 i surged at night.

I chose not to go in for the IUI. Good thing, because my day 21/22 progesterone level was 1.3, which is less than the 3 they use as a cut-off for ovulation. So, they tell me that I did not ovulate. So I probably didn't ovulate the entire last 2 or 3 cycles.

I just don't understand why. I am not really overweight, 5'0", less than 110 lbs (a gain of 8 lbs in the last 2-3 months). I've looked at the symptoms of PCOS and I don't fit a lot of it... My sugars are pretty well controlled with no insulin need. No excessive stress (nothing more than what I have had over the last year). No significant CF exacerbation.

I wish I had them check for my androgen level just to make sure it isn't PCOS.

Any suggestions? I know we don't ovulate every month, but this seems to be going on longer than one month.

My RE is going to talk to me after the holidays... I may do the lowest dose of clomid to stim and see if any eggs are produced.

Hey Jenn,
I'm usually the one with a million questions so I'm really no help when it comes to helping you figure out whats wrong.
But you said "I know "we" don't ovulate every month" you mean us CF'ers? I had no idea we don't ovulate every month. No one has ever told me this. How come we don't?

I think you need to do two things. First, it sounds like your fertility clinic isn't following you close enough. Why would they even suggest yet allow you to consider IUI when your cycles are so erratic. There would surely be a failed IUI attempt.

Second, you need to be on Clomid or Metformin. Sometimes they use only one yet other times both. Metformin matures immature follicles while Clomid ensures the release, I think...I didn't go that far, but they were suggested for me.

You clinic really needs to pay more attention to you and your needs especially if you are going to be taking drugs.

You may also want to get a second opinion. I actually got three!!! Did anyone bother to do an ultrasound on you? By an ultrasound, they could see if your ovaries are harboring immature follicles thus no ovulation would occur or very rarely? Even if you are ovulating, your eggs aren't capable of being fertilized if immature.

Your path sounds similar to mine except my cycles were like clockwork (every 30 days). I don't have any excess hair, obesity, or lack of periods. However upon ultrasound, my ovaries (moreso the left) was polycystic with many immature follicles. Also, my androgen, LH, and FSH levels were elevated. That was in July 2007.

This is where I get lost as far as I'm concerned...In September 2007, I started taking Vitex which has very similiar properties than those of Clomid. It is the more natural approach; however, you can not take Clomid and Vitex at the same time because they cancel each other out.

Anyway, I was on it for 5 months. Both reproductive clinics told me that my problems were PCOS and cervical mucus. The second clinic never retested me. They based all their explanations from the first clinic...

In February 2008, I met up with a new high-risk OB/GYN doctor who repeated all the bloodwork. His finding were completely different from the others. My progesterone level on Day 19 or 20 was 10 which was great. My LH and FSH ratio was fine. My androgen level was also fine. He didn't do an ultrasound but did do a laprascopy in May 2008. He concluded that I DID NOT have PCOS. His only reason for our infertility was cervical mucus and suggested IUI medicated or unmedicated. I still remember him saying, "Who told you that you had PCOS?"

Since my husband was against infertility methods, everything was dropped...I quit the Vitex...

To this day, I really don't know if the Vitex cured my PCOS or what. Vitex does indirectly help with PCOS but takes 4-6 months to kick in. I was on it 5 months at the time. However, I did get my periods so I wasn't as badly effected as others with the disorder. Maybe if I had given it more time with Mucinex and/or PreSeed. Who knows...

I know some of it is because you aren't ovulating, but I would ask anyway...
