My 10 year old nephew is newly diagnosed and has been through 2 months of TOBI, cleared his pseudomonas and now it is back. Any advice or suggestions on how to help prevent this- especially in school? Thanks

The pseudomonas may not have been completely cleared. Cultures are so inexact that they can be missed. I don't have any great suggestions for erradicating it, that's really something the doctors know. I hope that maybe inhaled tobi and iv antibiotics can help clear them out.

Like Heather said, cultures are not conclusive. Depending on what part of the lung the culture comes from, bacteria can easily be missed.

I also wanted to add that pseudomonas cannot be obtained at school any easier than it can be obtained at the grocery store, or even in your own kitchen. It is impossible to avoid. The best defense is teaching excellent hygeine. You are going to hear scary stories of pseudomonas that shows up in showerheads, sinks, etc. He cannot live in a bubble, but you can do your best to keep his environment as clean as possible.

I have realized that he probably colonizes pseudomonas, meaning its there to stay. He is on TOBI every other month, and when he is working up an exacerbation, we add oral cipro to the TOBI. If that doesn't work, we move on to ivs. TOBI and cipro have shown to be a good combo for many patients to avoid ivs, but sometimes even that can't avoid the need for some iv meds.

I have realized that he probably colonizes pseudomonas, meaning its there to stay. He is on TOBI every other month, and when he is working up an exacerbation, we add oral cipro to the TOBI. If that doesn't work, we move on to ivs. TOBI and cipro have shown to be a good combo for many patients to avoid ivs, but sometimes even that can't avoid the need for some iv meds.
