question about pseudomonas

my brother was at hospital the docs said he has pseudo this time,but they wanted to check it again,as my brother was at hospital for 15 days,they said it is possible due to this the check to show pseudo.the next time they didnt find pseudomonas only mycobacterium.they put him on third check.we are home now,he takes no antibiotics,we are waiting the results now.After the hospital he was coughing very much but he is better now.he started the nebulizer now,at the age of 26,for the first time,he didnt have much problems with lungs until now.what might have happenedwith the pseudo?are there any signs of it?it is dangerous waiting so much without meds?(its about 3 weeks for 3 checks)

If I understand what happened/your question correctly: Your brother cultured PA, then was clear for 2 checks, and is now waiting for a third culture to make sure it is gone?

Unfortunately there are no 100% guaranteed answers here, but yes it is very possible that he had the PA, was treated with antibiotics in the hospital and it is gone. If he is 26 years old and didn't have any lung problems until now, that is likely to happen.

It is also possible that the PA is still there, just in a different part of the lungs - cultures can sometimes be deceiving because if what you cough up is from a clear part of the lung the results will be negative. But again this might be more likely to happen to someone with more lung involvement than he has.

Did he do any inhaled Tobi treatments? The standard treatment for PA (psuedomonas) is Tobi and Cipro for one month. If he was treated for PA, doesn't have any symptoms and he is now been clear for two checks he probably doesn't have it anymore.

The real danger of psuedo comes from Long Term Infection and the type of psuedo. If this is his first experience with it and lung involvement, I wouldn't be too concerned with the timing. cultures always take about 5-7 days -- they have to wait for something to grow before they can tell what is growing. It is hard to wait it out. What does sound kind of strange in this case is why did they question the results of the first one? Usually if you culture it, you get treatment for it. After 30 days of treatment you culture again to see if it is gone. If they found PA on the first test, why wasn't he treated?

I suppose it is possible the first test only showed a very small number of the bacteria so they thought it wasn't necessary to treat ??

My best advice would be to ask your brother to check with the doctor again and explain the process of what is happening to him again -- you two shouldn't have to be second guessing what's going on - the doctor should explain it to you again.

If this helps at all, I've had pseudomonas (PA) since I was 7 years old and I'm guessing there are many others here that have had it for years & are still alive too.

It's kind of hard for CFs to avoid PA, as it is every where. When I was younger, they only treated it when they felt it was "flaring up". Now days they take a much more active approach to battling PA, trying to catch it right away.
