These may be dumb questions but my brain is going round and round and I just thought yall could help. My son has been on Cayston for about a year but after a severe bleed he stopped for a few months- bad idea. He decided to really start being compliant so we called to reorder Cayston- cant get it because of break in treatment. They prescribed Tobi- he hasnt taken it in a long time. He should do saline first-is this right? And he uses same neb with saline and Tobi? Also I have read about people using masks to do saline and that helps sinuses. How do we get these masks? Do we have to have prescription? Thanks.

I do not do HTS or Tobi, as I don't tolerate either very well. I have been told by a friend of mine that a sidestream nebulizer is much better for HTS in terms of reducing irritation. And yes, I would do albuterol then HTS then Tobi.

When you say you can't tolerate are you referring to bleeding/irritation? or just don't feel well while on it?

Irritation, specifically bronchospasm. This is especially true of HTS. Pulmozyme tears up my vocal cords. Tobi is the lesser of the evils, but I have not noticed a profound benefit that makes it worth doing.

You can do HTS and tobi out of the pari neb but don't use the same nebulizer for both unless it is boiled in between.

HTS is MUCH more tolerable in the sidestream nebulizer than in the Pari. The Pari is a finer particle size and will tear you up and cause spasms in the airway. Do Tobi from the Pari though as it needs to be micronized to a size most nebs can't do.
