Spinning and dizzy.. RE Antibiotics?

Hi,

I am doing quite a few antibiotics IV at the moment. Tobra every 8 hours, Zosyn every 6 hours, and Zyvox every 12... I am doing them at home right now and what I have noticed today and yesterday is a fullness in my ears and a dizzy feeling.... RIght now I am still feeling dizzy and spinning...I also am getting kidney area cramping... Is this only a sign of having to much in my system? or can this be because this is just a side effect of the drug that is normal. I am not peeing as much as I usually do either.. though I am still peeing... I guess I will call me team tomorrow or page them cause I probably need a peak and trough drawn. from what I have learned about these drugs.. Do you think this means I have to go back in the hospital to get the levels worked out? or will they do this at home? and would you recommend I hold off on taking my next dose? I know you guys are not the doctor but you have more experience in this then I do...

I've never been on IV antibiotics so much, nor have I been on oral antibiotics, lately, but the description you made about dizzyness, ringing(in the ears?), may indicate the liver's working O.T. for those antibiotics. I've had a few cases which I had taken too much aspirin in a weeks time (I took the normal dosage by the prescription) that was too much for my liver to handle.
I'd highly recommend contacting your doctor for that purpose, he could at least indicate whether it's normal and will pass, or he had to make adjustments for what he's giving you.

tobra can be dangerous if the levels arent right, but my guess is they checked your levels because that is very important on tobra. when you are home they are supposed to check them once a week at least once they have been set.
HOWEVER, zosyn can have weird side effects..one being dizziness. i was on it last month and got a weird rash and although they did not say i had an actual "allergic reaction" they took me off of it.

these side effects dont sound good and you probly should tell them because dizziness and full-head feeling, etc, arent within the realm of normal/ok side effects, the way that mild nausea is, for example.

One thing that seemed weird to me is that you haven't been peeing as much. Normally you pee even more on IVs. You're not drinking all that fluid, but it still goes through you and you pee it out. If you're on that many, that often, and you're peeing less... that might make me wonder. I'd be willing to guess you're not in immediate danger, but definitely call the clinic tomorrow, or Monday, or whatever is the first opportunity.

p.s. sorry i think your home care company will def. be able to draw the labs for the levels...you should tell them you want that if that is what you want. also, if i were you i would only stop the drugs if you really felt that the sickness you were feeling might be doing more harm than good. i just usually try to gage what a nurse would suggest if i were actually in the hospital...and most of the time if you complain about something they might say, lets get throguh this next dose, and then wait and see, OR if you really dont feel like you can do it, then lets just wait to talk to the doctor.

I had the exact same thing happen while in the hospital on IV tobra. I had used it many, many times before, but for some reason this time I had a problem. My levels were fine, but the room spun & I was terribly dizzy like I was out all night drinking. It was a bitch to go to the bathroom. Everytime I sat down on the toilet I wold almost fall over. They didnt want to remove me from the tobra right away because my infections respond so well & tried to rule out other things first. It was an awful time & even after finally stopping the tobra it took a few days to get back to normal. I have used the tobra since then with no problems so we will never know for sure why the problem arose!

Ok Jenn... as you know, Tobramycin is metabolized by the kidneys, not the liver, and that's why it's so nephrotoxic. Anytime you have decreased output, you need to call doc immediately to have your levels checked and a chemistry. It may be nothing but some minor dehydration, but with all the drugs you are on, I wouldn't chance it. The dizziness could just be a side effect, or it could mean your levels are too high. All the way around.. it's best that you get a tobra level and a chemistry checked today. Call me.

Thanks so much for the replies.... Well I would have had a better night had I been wearing a crash helmet.... I had to keep getting up to get something.... like a flush or the next IV out of the fridge and everytime I did.. I walked right into a wall.... I have a nice lump to go with my bruises now.

I ended up calling my home nurse ppl to which they said half asleep, " Go to the emergency room to get checked" I know, being that I work in an emergency room that that is the standard to say. but then she told me the Next option she said I wasn't due to have my levels checked since I just have been out of the hospital a few days... she said try to get through your next dose and if the problem persists call your doctor.

Anyhow, as I woke up this am.. I am less spinny but I am now dripping again (Zyvox) and my ears are starting to hurt...also I am not sure if this is related... but my blood sugars have started to sky rocket again, even though they were on there way back down when I left the hospital..I am 300 right now and I didn't even eat anything yet.

As a side note... I have learned something about myself.. HAHAHAAHA Who knew what a multi tasker I could be.. I am doing a gravity drip, nebulizing, vesting and doing a post at the same time.....and watching TV ..... and check my sugar levels... WE ARE such talented ppl..

Today I am going to write down how much I actually pee and make a real effort to drink, I should be able to gauge what is going on from thay, Like Emily said as well I am on so much fluid, ( one of my IVS is actually 300 mls at a time) I think I should be peeing more. I used to get up in the night before IVs and pee, now I am going maybe three times a day.

be careful of ototoxicity! tobra has destroyed my vestibular system, what gives you the ability to balance...it's an inner ear thing. i had been on tobra iv so many times that i think it finally took it's toll. one of the things i noticed was fullness in the ears and a loss of balance that has ended up being permanent. having those symptoms is not something to take lightly. i was on tobra and had similar feelings a few months prior to the time it destroyed my vestibular system, but the effects went away so i thought nothing more of it. the next time i was on it 3 months later the effects became permanent. i have gone through physical rehab to learn to balance again by using what i feel in my feet instead of relying on the inner ear, but i still am off balance quite a bit of the time and it really sucks!

sounds to me that it could be tobra, cause it causes ringing in the ears and will give you kidney infection, so Id stop trobra If I was you.
