Travel to Japan with Hill Rom 105 Vest

We have traveled within the United States with my daughter's Hill Rom 105 vest and we want to travel to Japan for Spring break. I read many of the travel postings and I am confident that the vest will fit on the plane. Has anyone taken the Hill Rom 105 vest on a bullet train in Japan. I read that you have to ship larger bags and can only take carry on luggage on the train. We will have 4 people and four small roller bags, plus the Hill Rom bag and a backpack.
Does the Hill Rom bag fit as a carry on? Are the people in Japan friendly to medical equipment not counting as a carry on?
We could also stay in one hotel and then leave the Vest bag at the hotel and take a 1 night excursion from Tokyo to Kyoto and then back to the hotel. Do you think the vest would be safe for a one night stay at a hotel without us.
Any suggestions would be appreciated. We want our daughter to see the world, sowe just need a few pointers on how to make that dream happen.
