Vest before or after a meal?

My dd, age 9, has been at the same weight for the last two years. We've tried many things to get her weight up. Everything works for a little while, then the weight goes back down. She's been stuck in this 4 pound window of weight way too long for a kid. She's not sick. She just had a bronch and sinus surgery without culturing anything nasty. Her PFTs are fine.

As I tried to figure out what changed two years ago, I realized that she started using the vest two years ago. We have her do her vest treatments before breakfast and before dinner because we thought it would be more comfortable on an empty stomach.

Could using the vest upset her stomach and effect her appetite? Should we have her eat first? Any problem using the vest on a full stomach?

Any other ideas for my particular and particularly slow eater?

With DS he never had issues doing CPT or the vest on a full stomach. And I'll admit, I sometimes let him eat -- have a snack while he's doing his vest. Drives me nuts but he claims that he's soooooo hungry! <img src="i/expressions/face-icon-small-smile.gif" border="0"> We do his vest right away when we get home from school and before dinner because otherwise we get delayed and then he has to stay up too late doing his bedtime vest/neb treatment.

He's also a VERY slow eater. Part of it is that he gets distracted -- he's so busy chattering away. Or on the weekends when we let him eat lunch in front of the tv he becomes hypnotised. If we go out to eat, he'll focus on his food and eat so much we're shocked.

DS is also a huge snacker. So I try to push high calorie things like peanuts, cheese & crackers. On the way home from school he can easily down 3 packages of fruit snacks.

Have you tried things like high calorie drinks? I know my center likes to pass out Ensure and Boost, personally I could never stand the taste. I drink Resource Health Shakes, they're 300 calories per container that's about the size of a small milk carton like they get at school. I drink 2-6 a day and have done so since I was probably 4 or 5. I'm pretty sure you can order them through Walgreens, we get ours through GFS. I haven't had problems with vest decreasing appetite, I usually try to avoid doing aerosols/vest after eating because if I get coughing too hard I usually end up throwing up whatever I ate.
