To those of you who have a 504 in place, can you tell me if you had to sign a document giving the school district rights to your child's medical records? I am very uncomfortable with this, as we have had numerous problems with the administration in our school since moving to North Carolina. Basically, I want them to have as little info on my children as possible, yet want to make sure Andrew does not get penalized for absences, etc. Is it necessary that they have access to our doctor? I understand needing a letter from the hospital to confirm his diagnosis, but school wants me to sign that it is ok for them to have oral and written conversation with our doctor about Andrew's health records and appointments. Have any of you agreed to this?

i dont know about this but the way you describe it, i wouldnt be comfortable e ither! i would make sure that they could confirm appointments and what have you, but there is no reason the school should need access to the actual medical records.

The most they can demand is a letter from the Dr stating that Andrew has CF, and what is required of them. IE: drink with him at all time, enzymes at school, occasional absences for appts & tune-ups, etc.

Do you go to a CF center? If so they should have a social worker, involve them and they can guide you and also educate the school as this is part of their job.

I am not sure what a 501 is, I am sure that each school district has different procedures. My sons school is aware that he has cf and I just send in a note and keep in communication with the school. they do not need full access to his medical records, maybe just a note.
