504 plan for 3rd grader

So, My daughter with CF is in 3rd grade and we do not have an official 504 plan set up. My daughter had a winter from H#ll being sick in 2nd grade and I don't want a repeat for 3rd grade.<br>My only request for her 504 would be for her to have a tutor in place if she is sick and absent for 3 or more days in a row. We did not have this in place before because the work was doable with me tutoring her and my daughter is very smart so she quickly catches up. <br>She is in a high reading level and math level and I want her to stay there.<br>Anything else that anyone can think I am missing?<br>Everything else is fine, she takes her enzymes herself, has an inhaler with the nurse.<br>It's just having a tutor really it would be mostly for math, since math builds on itself.<br>Thanks in advance!

I had a 504 plan when I was in middle school but that was a long time ago so I can't really remember exactly everything that was written in it. From what I can remember though, I think the school allowed me to have extra time for some assignments. For example: An assignment/project would be due in 3 days for the entire class but with the 504 plan for me it wouldn't be due until 3 days later. A few extra days. Sometimes even a week. BUT, I will say to be cautious with that method. It caused me to fall behind here and there. Your daughter is in 3rd grade though, I would think the work load isn't too heavy. She sounds very smart. I don't think she would have a problem with too much work until later years. Something like that might be useful in the future with higher grade levels. Just to put that out there.

My son is in the 3rd grade also and we just updated his 504 a couple of weeks ago. <br><br>His included:<br><ol><li>extra time to make-up assignments</li><li>assignments provided ahead of time for hospitalizations/dr's appts</li><li>fluids/snacks in class as needed</li><li>unrestricted access to the bathroom/nurse's bathroom</li><li>contact family for outbreak of flu or other communicable disease</li></ol><br>Next year I am going to request early dismissal for afternoon vest treatments when cough/cold present. His school does not end until 345pm, and you are not allowed to sign out after 3pm. He just developed a cold over the weekend so he is not getting his extra vest treatment until 4pm - would much rather it be at 315pm. <br><br><br><br>

I am the 504 coordinator at the school I work in. I agree with scrapper. what state do you live in? Not only should extra time should be listed like this:

extra time/alternate setting for all tests, projects and homework, including state assessment tests.

i would also add water bottle to be available thoughtout the day.

what about gym? is your child able to participate?

if you want the tutor the 504 plan should also say:

tutor to be provided when student has been absent for three consecutive days or more

Thank you for your input! I need to get this officially set up. Most of these things are in place but not in an official 504 plan. The school has been fine with accomadations needed. The issue I had is because my daughter is performing at or above grade level; requesting a tutor gets me looks of surprise at first. My point is why should she fall behind if she gets sick.

She can do gym class fine.

When she does get sick; she usually misses some school, then does "half day". Which means she misses out on gym, art music, french..... The meds she is on when she is sick make her feel sicker before she feels better, so she feels like a space cadet in school.
