Advair timing?

Hi - Just wondering when in the med sequence those of you that use advair do it? Jason used to neb alb. and pulmicort, then pulmozyme, then do his accapella and then on his on months do TOBI. This is all done twice a day. At his last clinic, they told us to stop the pulmicort and try the advair. They still want him to do his alb. in the neb and just do the advair inhaler after and then proceed with the rest of the treatment. Sometimes he is half sleeping when doing a treatment and obviously wouldn't be able to do an inhaler at that time. I was wondering if other than keeping the doses approx. 12 hours apart if it def. had to be during after the albuterol. Thanks in advance for any input. My doctor told me that I could do my advair at the very end of my treatments (after xopenex, pumozyme & TOBI), or I could do it after pulmozyme...basically she said that the most important thing is that it is done after albuterol/xopenex (same thing) so that the airways are wide open & it can do it's job more efficiently that way. Was told to do the inhalers first, cause then they will open the air ways for pulmozyme for it to do its job, by thinning the mucus, then do the vest which moves the mucus around and psossibly out. That was what I was told, but then again I dont take the other two things you guys mentioned.

I asked the same question to both my doctor and the pharmacist. My doctor said i can do the advair first, and my pharmacist recommended i do the advair first also. I started out using the advair first right from the start, and always have. At one point i tried using the albuterol first and there was no difference at all. Thats when i asked my pharmacist and he recommended the advair be done first. Just be sure to always rinse after using the advair to avoid thrush.

I never much pay attention to what order I do the stuff in, as long as I'm *doing* it. Currently my routine is in the morning: Nasacort, Albuterol, Advair.

Oh I'm a liar, at night the order I do it in is: Pulmozyme, Nasacort, Albuterol, Advair, and then my CPT. Woo. I messed that up.

I do it at the very end of everything: Pulmozyme, Albuterol, Acapella, Colistin, then the Advair, and finally my nasal levage and Flonase. At night it's the same, minus the Pulmozyme. My doc said to do the Advair last so it gets down deeper into the lungs.

I take advair first... and after reading the posts this makes no sense to me what so ever.... It totally makes more sense to do it in the end because you are the most open then... I will have to switch things around... I do take high doses of oral steroids daily so I don't really have a need for the steroid part of the advair but the other part is good ....

Kait does advair and vest in the morning in the afternoon she does albuterol and vest around 4:00p.m. and bedtime around 9:00p.m. she does her advair, vest and then her pulmozyme. She has always done advair first and pulmozyme last. when she is sick we increase her vest treatments to 4x daily, we also do her albuterol every 4 hours.

same here (taking Tobi last), ok, what does Advair actually do for ya cause after reading all these post it seems that it does more than what i know, all i knew was it opened the air ways.

It is funny every doctor has a different opinion on the order of meds. I am sure that all doctors would agree to do your inhalers after your nebs. My pulmonologist suggests. pulmozyme, xopenex then tobi. Unlike Debra's doctor, my guy thinks we should thin the mucous with pulmozyme, COUGH, then bronchodilate(xopenex/albuterol), COUGH, then tobi, COUGH. Flutter/CPT should follow, then COUGH. After all of this then inhalers.
I really think the xopenex and pulmozyme are interchangeable depending on your specific case. If you have hyperactive airways then maybe you take the xopenex first. But if your problem is more thickened secrtetions and less bronchospasm then maybe the pulmozyme first. Either way the rest of the order is pretty much set.

My daughter does pulmozyme before chestPT (In fact we have to wait 30 minutes between pulozyme and CPT). The pulmozyme is supposed to help loosen the mucus so it is more easily removed during CPT). We still use percussion and postural drainage, so maybe using the vest makes a difference. Anyway, it is very interesting that everyone seems to have different orders.

i was always told to do the tobi or colistin(whichever im doing,i rotate them) last after the others and cpt so that way it stays in the lungs to do its thing rather than get coughed out of the lungs during cpt

While in the hospital , the respitory theripist has Kait do her pulmozyme while she is doing her vest. ???????

Everytime I see someone type in acapella- I think of the old Andy Grifffin show when Andy asks Barney if he can sing acapella and he breaks into a really bad song "acapella acapella" lol- the world would be a better place if everyone watched this show before going to sleep- lol. Just a funny thought.
