I had to do the same. We went through 3 pre-auths, then my dr finally wrote them a letter of necessity for it... though my pharmacist was not the brightest and i had to argue with them about the way to admin it and what i needed from them.

my insurance kept denying it because it was an "injectable medicine" even though I was prescribed it by inhalation. I sent in appeal after appeal after appeal, even going so far as to send them research studies about the benefits of inhaled colistin in pwcf. I had my doctor write letter after letter, and still it was not covered. My new bacteria is resistant anyway, so I'm on inhaled amikacin now... but I never got it approved. UGH. Insurance companies make me so mad sometimes with the hoops that they want me to jump through.

Is it through IV or neb? I do the nebulized colistin and I have Medicaid. The nurse has to call and argue with them to get them to pay. She has to get a "prior authorization" so they will pay. I am on it for a month then off for a month but for them to pay I have to pick up the prescription every month. My husband has me on his insurance through his work for what Medicaid won't cover. His insurance won't cover most of CF medicines period.
