Boogers! DS was so proud

Hi Liza,

It does take 48 hours to get a preliminary respiratory culture; usually we get our final report in 5 days which includes; the bacteria type and sensitivities. What I have seen on the48 hour report may be "normal flora" if there is absolutely no bug, but Maggie is usually growing something. So the 48 hour report may say staph aureus, but we don't have the sensitivities back. Or it could say gram negative bacteria without sensitivites back yet at 48 hours...

Rarely however, she would have normal respiratory flora, but we never would get a calltil 5 days later; correction I call at 5 days and then they tell me...

Maybe it was normal flora ; that would be good?

How is he feeling?
He feels fine. He developed a head cold last Thursday, but fortunately it seemed to be clearing up.

I'm just so cynical about his peds doctor -- the head of the local CF clinic --- because during the appointment he questioned DS being on Zithromax -- that it was usually only prescribed for those who've cultured pseudo.

And who knows, maybe it is just normal flora. Past experience though I get a similar call and then receive a report in the mail indicating he's growing other cooties. Just seems odd that they'd call this one the final report based on the initial findings.

Final report in less than 3 full days? That doesn't sound right to me. Preliminary report, maybe, but that is not enough time to finish growing out for bugs, nevermind determine sensitivities. Emily's prelim have said normal flora at a couple days and then grow out stuff as more time goes by.

Zithromax used to be given primarily to kids w/PA. Emily's team talked to us about it when we started it about 6mo ago though and said that at the last couple big conferences, they'd arrived at the consensus that it could benefit most children, whether or not they had PA.

I think I read that the zithromax acts as a anti-inflamatory much the same as ibuprofin therapy.

The thing about the zithromax just flabbergasted me. Head of an accreditted CF clinic and doesn't know that it's used for inflammation issues. One of their doctors goes to the NACF conference every year, but they are just sooooo backwards.

DS' goes to him twice a year for well child visits, labs, vaccines and throat/sputum cultures. And so we have a rapport with a local doctor. But I just shake my head at times.

When I questioned the results, the nurse said it was the final report. Ugh!

I received the report in the mail. It was dated Tuesday, the day after the culture -- final at 8:46 am. So pretty much overnight.

2+ few WBC, Squamous epithelial cells,
1 + rare gram negative & positive bacilli
3 moderate moderate gram positive cocci in pairs chains and clusters.

Ugh. It did indicate they'd keep the culture for 5 days as there was insufficient growth after overnight incubation.

Dr. Doofus also had that DS was in 2nd grade.

He did grow .1 inch since his appointment last month and did gain a pound. So 49th percentile for height and I think 57th percentile for weight.

Um, no wonder they had to keep it for further growth!

<begin quote>moderate gram positive cocci in pairs chains and clusters.</end quote>

Does he usually grow out staph or strep? Those are cocci commonly cultivated by our kids.

<begin quote>1 + rare gram negative & positive bacilli</end quote>

Bacilli (rods) can include pa, steno malt, h. flu among others.

Epithelial cells are the skin cells they almost always get along with the other stuff.
I'm glad to hear there was some weight gain- I know there was some concern there over the last several months.

I bet if I pull previous reports the initial findings will be the same.

I suspect it's still the steno maltophilia. He's never grown out staph or strep. And a year and a half ago he grew out a pseudomonas florensen (sp). Guess we'll see if we hear from them next week with an addendum. Just makes me shake my head.
