Call in a script

My dd developed a cough yesterday, that is getting junky today.
I called the CF center and they called in a script for 21
days of Amox.  Thus far she hasn't had any lung issues, but
did culture staph at the first clinic, second clinic her culture
was clear.  I told the nurse and doc that I have the same
cough going, but that I just wanted to check in.  They said
they want her on the antibiotic.  Is it normal for them not to
bring them in or listen to them?   I'm thrilled at their
responsiveness, but I don't want her to become antibiotic resistant
this young.  And I understand that many people live quite far
from the clinic, but ours is in town.  So why don't they want
to see her?

I have found my clinic to be not overhelpful in seeing u for a
cough or they refer us to the pediatrician.  My son is 9 and I
can remember when he was younger and got a cough thinking all my
God, it's going to go into his lungs, never did and never got
antibiotics.  At the time I was kind of mad that they didn't
give us some, but now I know they were right and thankful he has
been almost antibiotic free.  Kids w/o CF get colds all the
time, and CF kids can also get coughs that can go away on their
own.  I'm not a doctor, but I would not be so quick to put
your child on meds, give it a week, or I even went and bought a
stethoscope so that I could listen to see if it was in his chest or
all upper respiratory which a cough the majority of the time is
with drainage down causing u to cough.

I have a great relationship with our pedi and he will call in
things (antibiotics, etc.) for me for my toddler based on what
symptoms I tell him over the phone. I know that he isn't our CF
clinic, but I can imagine that if they are busy, they'd rather call
something in and see how she responds. They would probably try an
antibiotic first anyway, CF child or not. I know that probably
doesn't help, but they may also be thinking they are saving you a
co-pay and a trip out with your child (esp. if the weather is
getting colder). In my heart, I would have to agree with you...I
would want my infant seen about in the office just in case
regardless of if it was anything to "worry about" or
not.<br>

Once I got in with my docs office when I was younger they often would call in scripts for me for meds if I was mildly sick. If it progressed into something worse then we were told to call back and they normally brought me in the office for a visit.

I would say to take the meds as prescribed - there is a reason for them giving you the meds. Yes your daughter may not be very sick right now, but alot of the point in them prescribing meds and in you keeping them up to date with your daughters health is in hopes to prevent further infection and unneccesary lung damage or anything.

As for drug resistance. If you take the meds as prescribed and are not on the same med every other month or every few weeks then it should not be a problem. Also to add ... in the event that your daughter developed resistance to a med it does not necessarily mean it is permanent. I developed resistance to Cipro several times as a child because they always put me on it - when I say always I mean I would get infection every few months and they ALWAYS gave it to me. I would have a culture that showed resistance and I would be prescribed something else for say a year ... or even just 6 months - I would stay off Cipro - then poof I would have another culture and I was sensitive to Cipro again. I get the same thing even now as an adult sometime and I am not on Cipro that often either. It varies sometimes with different drugs and with different bacteria.

I know sometimes people get uneasy when they don't see the doc and the doc just gives them something - problem unseen - but remember they deal with ALOT of CF patients and they may be noticing a pattern right now. Alot of their patients may be coming down with something and they may be treating alot of kids with the same meds for the same symptoms. It is odd, but often CFs tend to get sick alot at the time - it definitely fluctuates. I would give the meds - the full course even if you notice improvement before they are all gone - and hope that the problem doesn't progress. It is better to me to overtreat a small problem then let it go and get even worse to the point it may not be easily fixed with oral antibiotics.

Sorry I will stop talking now. That is just my thoughts as a person with CF - not a parent.

Hope you and your little one start feeling better soon.

Thanks Lindsey, I appreciate your input.  I came to this site
to get information from other parents, and particularly from adults
with CF, so your insight is very helpful.  I'm glad to hear
that the resistance isn't necessarily permanent.  That does
make me feel better.  I have already started her on the meds,
and intend to follow the prescribed process.  I think I am
just not used to having a doctor basically just trust MY instinct
to know -- that he believes if I called then its worth dealing
with.   I've been questioning my instincts ever since the
diagnosis.  <br>
<br>
I do think you are right about many CFers getting the same thing.
When I spoke with the CF nurse today she said I was the fifth
parent complaining of the child and adult both having this same
cough.

Yes, I am surprised they called in a prescription after coming down with a cold yesterday - especially a three week course. I usually only call the cf office when I am really concerned about the cough. I usually call when I think they need to be on antibiotics soon or if they don't go on antibiotics the cough could lead to a hospitalization because it is that bad. Otherwise my kids usually catch 4 or 5 colds each year. Usually just extra treatments helps them get over it and they get over it without the need of antibiotics. My daughter has only been on oral antibotic once in the past four years. My son is about twice a year.
Hope everyone in your house is feeling better soon!

Like Sharon, I have never called the CF clinic for a cold/cough. I'm not saying that you shouldn't have. You know your child best. But I can honestly say that it has never crossed my mind to put Danny (or the other two kids) on antibiotics for a cold. There have been a few bad colds but I have always just taken the wait and see approach, with increased CPT, and he has always gotten over them on his own. Until reading posts on this site I never knew that docs would be so quick to give antibiotcs for this type of thing. Do your CF docs instruct you to call anytime there is a cough/cold? Even if everyone else in your house has it and its just clearly a cold?
