This has happened to me quite a few times. When you take the faulty needle out, if you look at the bottom, usually the needle is all bent up. I think sometimes I just turn it onto the pen too fast and mess it up.

The once in a blue moon that this has happen to me it was rectified with a new needle and I use the smallest mini ones in my Novolog flexpen.
