I have not spoken to the transplant doc yet b/c my insurance does not cover it at the hospital I go to ,St. Paul in Dallas, but Baylor in Dallas does. So the social worker said my doctor is going to call the insurance company to make a deal with them so we will see 1/4/05 what happens.

Gail,
Let me know it that works, just out of pure curiosity on my part. When my hubbie started on colistin I posted a question about how to properly mix it because I was given the directions I gave you (by the pharmacy) and the directions that my doctor gave me (the one you posted). I got an overwhelming response on this website that the mixture of the NS and sterile water work better for most. Just a thought.

I mix it with 2ml of water, that is what it said to do on the box. I am having a problem and wonder if anybody is having this problem too. I get my equipment set up put the colistin in the nebulizer turn the compressor on but nothing is coming out of the nebulizer so I check to see if air is coming through the tube which it is. Everything seems to be working fine but there is nothing coming out the other end. There are other times it fires right up no problem. What would cause this?

It is very frustrating to waste 20 minutes trying to get it to work plus the 30 minutes to take the drug.

If, in mixing the Coliston, you've created too much foam, that could be causing the back up. I'm not sure. But I know foam can be a real problem, so make sure that's not what's doing it.

I thought the same thing, so this morning I let it sit for few minutes for the foam to go down and it still did it.

sometimes the type of "acorn" (for lack of a better word) you use with the colistin can make a BIG difference. For my husband, the only "acorn" that has worked with the colistin is a brand called AeroEclipse nebulizer. It has a green button on the top of it that helps with the flow of things. We had the same problem you do, then we switched "acorn's" on advice of the respriatory therapist we talked to and haven't had a problem since. You might give that a try.

colistin is very sticky, be sure to rinse out your nebulizer after each treatment it will clog up, especially the circuits that utilize exhaltion valves. I have had luck with drawing up the drug and refrigerating it, it cuts down on the foam.

I always make up 5 viles and put them in the fridge, and I rinse out my nebulizer after every use with hot water. I haven't been able to take my last three doses (2 yesterday and 1 this morning) because it won't nebulize.

How do you mix yours dave? I have heard of 2-3 different ways on this site, but my husband hasn't had a problem. also, what type of "acorn" device do you use. His CF doc said there is a specific one to prevent problems such as you are mentioning.

I use the Pari Nebulizer (I don't know what you mean by achorn) I use a syringe and take 2ml of sterilized water and inject into the vile. I make 5 viles at a time and them in the fridge.

Dave,
The brand of nebulizer he uses is called Aero Eclipse. What I mean by "acorn" is the part where you put the liquid for your nebulizer treatment. it kind of looks like an acor, hence....acorn. I don't know what else to call it. The doctor has told us that we are to use the Aero Eclipse and only the Aero Eclipse with the Colistin. With the Tobi and Pulmozyme we use a different, common brand although I am not sure what it is called.

We also mix the solution different. We use 2ML of Normal Saline in the bottle to reconstitute it. We shake that up and withdrawal 1ML and put it into the "acorn". Then we use 3ML Sterile water and add it to the 1ML already in the "acorn". That seems to alleviate both the clogging in the tubing, and for some people the colistin causes other problems.

We get our nebulizer stuff for free through the navy, if you are interested, I could mail you 1 of our extras (my husband is off colistin for a while and we have an extra). If you are interested, so you can see if it helps before you buy it, email me at division902@hotmail.com.
