Confused about mixing Colistin

I just attempted to do my first dose of Colistin. I received my order from Foundation Care and assumed they sent me everything I needed. Admittedly, I didn't pay too much attention to what I was doing because it seemed pretty self-explanatory. I figured it was similar to the old days of mixing Tobramycin before they came out with the Tobi ampules.

The instructions say to mix the Colistin with 2ml of sterile water, then remove 1ml and add 2ml of 0.9% NaCl. That is where I failed to pay attention. Foundation Care only sent me the NaCl. So I added that to the bottle not realizing it wasn't the sterile water. It was only when I went to add more of the same thing that I thought it was odd.

So my question is this... Did Foundation Care fail to send me vials of sterile water or am I supposed to "make" sterile water by boiling water each time I need to prepare the Colistin?

I've never gotten colistin from Foundation Care so I can't say for sure, but yes, I believe you should have gotten sterile water too. I always got sterile water along with my colistin script. I don't recall ever hearing of anyone doing their own sterile water for inhalation, but I could be wrong.

On another note, back when I used to use it I only used sterile water, no NaCl, so they've got you mixing it differently. Must be the new and improved method. :)

I'd call the pharmacy and ask for the sterile water! Or you could call your doc and have him call in the sterile water to a local pharmacy.

I think you can mix it just with the sterile water....that is the way I have been doing it for years now. I also get my nebs through Foundation care. But to clarify things, you should call them.
