We started off with the little rubber cup when ds was about 7 days old, however, his doctor indicated we'd always have our hands and we should learn that way. So we used index and middle fingers. Pretty much retired the rubber cup, except when one of my inlaws did CPT because they weren't as sure of themselves and didn't do cpt on DS on a regular basis.

We never used anything but our hands 30 years ago. My son was just over 5lbs when we started to do physio and he fell asleep every time, obviously we weren't hurting him.
