Another thing, we were told that we would know if the albuterol was working if he wan't coughing as much after using it for 3-4 hours. When I do the albuterol neb along with his vest, should he still not cough? Should I worry about this??

Nathan use to grow both of those all the time, he still grows staph a lot. It is my understanding that the H flu is common for the younger ones and eventually they don't grow it, or at least not as often.
He probably will cough during the vest and "hugh coughs" but most likely the cough will or should diminish shortly afterwards if the albeterol is truly helping. However, if Nathan is growing something in his cultures at all and is having any symptoms, ie cough, Dr. Accurso almost always starts an antibiiotic. If he just finished one and the cough comes back shortly after stopping and he grew something, he starts him back on one again and does a longer course. I know that each CFer is different, and some doctors are really conservative with antibiotics because of the risks of overuse, but I would think with the time of year, if I were you, I would push the issue and call everyday the cough is not better, but that is just me. Plus, I know Nathan really well after doing this for 14 years and have a really good sense of when to ignore his cough and when not to ignore it. I guess what I am really trying to say is to follow your gut and be persistant with what your instincts are telling you unless they can give you a really good reason not to or you feel comfortable. As matter of fact, I almost always just email Nathan's doctor directly now and CC one of the nurses so that should he want something ordered, they can order it for me and are already aware of the conversation. The only time I call, is if it is emergent and I need an immediate answer that can't wait for emails to go back and forth, which is rare, unless I know that his doctor is out of the area. Then, I was given all of his phone numbers including his cell and home phone numbers and now I have all his numbers plus his email. It is amazing how the other staff responds to you when they know that you can go directly to the doctor and completely bypass them if you want too and they know that you will do it if you are not satisfied with their answers. If you have the ability to go directly to the doctor, do it, they don't give you such information if they don't want you to use it. I know I had it for a long time before I got brave enough to use it and now I have no problem doing so.

I think that the 'cough' was one of the toughest things to figure out for me and my daughters. Both of my girls have cultured h-flu (with no symptoms) and staph (with and without symptoms).

My younger daughter very rarely coughs, so when she does have a cough going (one that persists throughout the day rather than what CPT should bring up), I immediately call clinic. For her it is unusual and I know she needs to be treated. She cultures Staph, MRSA and sometimes PA.

My older daughter coughs a bit every day. A few years ago I thought it was just her 'normal' because she seemed to get a lot of post nasal drip coughing spells. And then it slowly got worst...but I didn't really notice how bad it got until it started keeping her up at night. She was admitted and put on IVs and after two weeks her cough was pretty much completely gone! But there was lung damage left behind. She has gone back to having a productive cough every day, throughout the day. I know that she is getting worst when she can't sing or laugh without coughing. That's when I call clinic and we start the oral antibiotics. The orals normally knock the cough back a little. She cultures MRSA and Staph, never has cultured PA (which I was sure would happen after her little sister started culturing it, but almost two years later and still nothing!<img src="i/expressions/face-icon-small-smile.gif" border="0"> )

I think it really takes time to learn the differences in your child. They are all different and their CF will act different in even siblings.

Oh, and my nephew with CF who's disease is much more advanced than my girls, doesn't cough all the time (and he cultures everything!!) but when he is worstening, his cough is very harsh and sounds/looks like it just hurts. It's just different.

I know it sounds funny, but DS's breath smelled funny about 1.5 months ago, and while in for just a weight check I had them culture him because of this (kind of like a fishy smell). This is when he started culturing the H.Flu and he's smelled like it on and off ever since (more the past couple days).
Probably nothing, but had to ask <img src="i/expressions/face-icon-small-smile.gif" border="0">
