We moved down there for the waiting period prior to transplant and had that understanding that I would not leave New Orleans until I got the go ahead form the doctor. I ended up living in New Orleans for 5 months prior to the transplant and was authorized to leave 4 months after my transplant.

For the first year, I wanted to be followed by my team in New Orleans. After that, and when I had a serious pneumonia, I decided that transferring my care was the best decision. I'm now followed at UPENN. My doctor's know each other quite well from conferences and such. I'm in constant contact with both and my doctor from UPenn doesn't mind that I continue to lean on my old doctor for a second opinion.

So in short, yes, you can have the transplant in one place and set up post transplant care in another.
