The Onion is most definitely a joke. Example headlines on the website right now include: "America's First Gay President Concludes 2nd Term" (in reference to Bush), and "Housing Crises Vindicates Guy Who Still Lives with Parents." It's a satire newspaper.
As for the article, I think it was poking fun at how hard the CF charities have to struggle to raise funds. By basically saying "look, just because this is a fatal disease that affects 30,000+ people in the US, that doesn't mean we want YOUR help", the article is calling attention to how serious the disease really is and calling attention to the fact that not enough people support it. It's saying the opposite of what's true, claiming that everyone knows about CF and is willing to help (to the point where the CFF is having to turn down federal aid b/c we don't need it), when in fact barely anyone has heard of CF and we don't receive federal funding b/c it's "too rare." I think it's a great article, and the Onion's readers would understand that it's fake, as is everything in that paper.
