Maggie did not like the HS when we did it in the doc's office. She was trying to show the resp therapist how she could breathe in the medicine, and was breating really fast, kind of showing off. That really got Maggie coughing, ususally she doesn't mind her nebs at all, but she kept wanting the HS to be done. We also do all her nebs with a mask atachment at home to help her sinuses. At the doc's office they used a mouthpiece, so maybe when we try it at home we'll use the mask, plus have TV for distraction. Anyway, we'll see how it goes, I noticed this morning Maggie had mucus from her nose that came out, not to be gross but I wonder if it was the HS that helped get it out? THe resp therapist also said to use albuterol treatment before the HS to help with the coughing.

My older daughter (who just turned 9 last weekend<img src="i/expressions/face-icon-small-smile.gif" border="0">) tried HS in clinic last year in the PFT lab, and didn't tolerate it well. Her FEV1 dropped into the 30s from the 60s a few minutes after inhaling it. So her dr. put her on Pulmozyme.

I did notice that when we went to Cancun last year (we live in Missouri--far from the ocean <img src="i/expressions/face-icon-small-smile.gif" border="0"> ) that Kayla coughed a lot! She had no cough prior to going (had just come off IV anti) and the cough was gone once we came home. Obviously being so close to the salty ocean and playing in it as well...was playing a factor. So I can see where HS could definately help in clearing the mucus from the lungs!

I do hope her dr. has her try it again sometime, instead of the pulmozyme perhaps.

Albuterol First, (Ideally wait 15min before the next step), Then HTS, Then Percussion, Then Pulmozyme. I'm not sure when the other meds fit in b/c that is what we are on. The vest is not to be used after Pulmozyme nor at the same time (it breaks us the medication and makes it less effective); so your only option is to do the vest before Pulmozyme. I believe that you can use the vest at the same time as Alb and HTS.

Aaargh! I am reminded of why else I was skeptical of HS. We don't currently do Albuterol and I remember reading somewhere that often you need it when doing HS. Yet another med!

My dr. has me doing Xopenex, pulmozyme, vest -this is where I wait 15 mins. between pulmozyme and HS recommended by my dr., then I do my Advair and lastly, Tobi or Colistin. Is there anywhere that actually <i>says</i> what the official order should be? My dr. wanted the pulmozyme in first, so it would have time to work before the HS, which he was worried about affecting the effectiveness of the pulmozyme.<img src="i/expressions/face-icon-small-confused.gif" border="0">

My understanding of pulmozyme (Amy can probably do better on this) is that it's NOT fast acting. It needs to get in there and sit and soak before it can get to work. So it needs to be done after clearance so that we don't cough/exhale it all back out.

I think the HTS hydrates all the top layer of mucus that's like the equivalent of turning a power house on dirt. And once it's all hyrated and we do the clearance it gets the whole top layer of junk off and then the pulmo gets in there and sits and it slowly jack hammers all the serioussly cemented mucus my chopping up the psuedo at a dna level.

I always think the HTS is clearing out all the crap I made in the last 24-48hrs and clears a path for the pulmo to get in a rip up on the older mucus that has set up shop on a more long term basis.

That was long and I think I repeated myself...splendid <img src="i/expressions/face-icon-small-smile.gif" border="0">

I guess I'll give it a try both ways and see which one works best for me, and then let my dr. know that too <img src=""> . Ideally, I guess it would be get up in the morning and do my Xopenex, vest, HS, vest, Pulmozyme, vest, advair, and inhaled antibiotic, repeat, and go back to bed at night- ARRGGHHH! and LOL! <img src="">

Now that my meds take twice as long, I have to keep reminding myself that it's better to lose a few hours during the day, than to lose the whole day (and all the following ones... <img src="i/expressions/face-icon-small-wink.gif" border="0"> )
