For those of you whose child cultured Pseudomonas for the first time did you use IV Tobramycin to try to eradicate it?

Maggie's followup goes like this: she was recultured IF her culture comes back no PA that is GREAT!

If her culture comes back with PA we have the option of being admitted for a PICC IV Tobramycin for 2 weeks to try to eradicate it OR to just do TOBI 28 on 28 off.

We are leaning towards the IV Tobramycin IF she still has the PA in her culture.

We will find out next week of the results...Just trying to plan...

Right now she is better, just a slight runny nose PFT's up, o2 sat 98%.

She feels good off the CIpro and TOBI and Zyvox...

Alyssa's never cultured PA but has been on IV Tobra 4 different times. Its pretty broad spectrum so they give it to get her staph and any PA that might be hiding.

I was told that its best to hit the PA with both IV Tobra and Tobi because there is a need to hit it systemically (IV) and locally (TObi). There can be spots in the lungs or sinuses that don't get the TOBI in, so the IV gets to that. There is also a possibility with sinus blockages or other issues that might keep the IV getting in, so the TOBI doubles it up.

I hope this makes sense and helps. I just know that I asked this very question to the docs on one of the last hospital stays.

I hope Maggies culture is clear and you don't have to do IVs. But if it isn't, I would suggest doing both.

Thanks Heather. We are leaning towards being more aggressive with the IV tobramycin. However, I am not keen on having her admitted during RSV, flu season. I guess I can't control everything.

I am upset cause she has been on antibiotics, alot this winter and I feel her body needs a break.

Hopefully, the culture will be clear and we can dodge this bullet but I like getting my ducks in a row.

How many times a day is Tobramycin?

Tobra has always been once a day for us. In Dec, they admitted us at 8am, placed the picc, started the Tobra (and timentin) waited 6 hours to get blood levels, kept her overnight to make sure she did okay on all the meds, and then let us go home on the IVs. It was our quickest stay, and it worked well for our life and it kept her out of that place during flu and rsv season.

My point is that you should ask if this is possible. It was our docs idea, but I was very pleased.

I just wanted to agree with what heather said, for me tobramycin has always only been once a day and they like me to do tobi at the same time regardless if it's my normal "on" part of the cycle

hi my baby is six months and was positive to pseudomonos im treating her at home with cipro and tobi, i hope this will eradicate this bug

The first time I cultured PA, I was put on IV Tobramycin for 14 days. I think it is definately best to go the home IV route. Good luck with everything.
