Saline is extremely beneficial to CF care, so no it doesn't choke you. I used the IPV twice a day with HTS and an additional treatment with regular .9% saline from the drugstore. The problem with using HTS in the IPV is you have to have a minimum amount of saline in the neb for the machine to work, so you end up wasting expensive medication. Most people prefer to use the IPV with regular saline and do the HTS as a regular neb, if needed.

The machine is somewhere between a neb and a percussor, really. It looks and functions like a neb (you put the mouthpiece in and breathe in liquid), but instead of "misting", it expells the saline in quick bursts. You can adjust the frequency and intensity of the bursts, similar to how you control the settings on a vest. Each burst essentially forces air and salt water into your lungs, which causes a precussive reaction. This is why I think it's not recommended for those with issues like lung collapse -- because it vibrates the lungs from the INSIDE (rather than shaking them from the outside), it can cause additional irritation and air trapping. But it does work wonders for a lot of people. I know several CFers who won't use anything else, and I really like the machine. The main reason I switched back was because I was using the machine so many times a day that the saline was starting to cause issues with my throat. I do like the vest, but if I'm super junky I will always add an IPV treatment.
