Meds & Pregnancy

My husband & I have decided now is the right time to start TTC. I'm the healthiest I have been in over 3 years. My lung functions have actually IMPROVED!

Now I'm concerned about what medicines could effect my baby when i'm pregnant & when/if I breast feed.
Currently, the inhaled meds I am on are: albuterol, TOBI, Cayston & Symbicort. Orally: Zithromax, Predisone, Nexium.

Obviously I plan on speaking about this with my DR. But I would like to know what all mothers with CF took & did not take when they were pregnant. Did you have complications? Did you breast feed?

Congrats on your decision! You're being smart and planning this all out which is the best way to do it. Preg meds and breastfeeding meds actually are quite different. Some that r safe for preg just aren't as safe for bfeeding and vice versa.
For my first and 2nd (current) preg I was on albuterol, pulmicort as needed, hypertonic saline, pulmozyme, zithromax, and pepcid. I use ceftin when I get sick. Most" extra" meds like zithro, pulmicort etc I tried to hold off on until my 2nd trimester but that was a personal decision. I was assured they were safe but wanted to be as protective during the first tri as possible.
in my first preg cayston didn't exist. My docs have said it is very safe for this preg, but im saving it for if I get sick.
Fyi from what I remember nexium may not be the best preg choice, but don't quote me. Just check it out with the docs first. I also have acid reflux and am holding off with over the counters as long as possible, then ill switch to my old protonix.
I breastfed till 11 months when I had an exacerbation and needed stronger ivs not good for bfeeding. My goal was 6 months initially so I was so happy.
When u discuss preg with your docs, they'll likely review all your meds and safety. I had a genetic counselor who specializes in preg medicine review my meds. Most stayed and some were switched. I got a 2nd opinion from a qualified obgyn also.

I did take prednisone late in my preg during an exacerbation. Its more of a drug for when u need it and not a daily drug so personally id address that, speaking of pregnancy. I did not take tobi. Most obgyns, and many cf docs do not consider tobi safe for pregnancy. But some do depending on how much the mother needs it. That will b up to u and your doc. Regardless of what u do, my goal was to be healthy enough going into pregnancy to not need tons of meds. But they were there for me when I needed them. Each situation is different. And above all, a healthy mom equals a healthy baby. Some of these meds are what's made it possible for us to have pregnancies safely now.

Bactrim is a C. not sure about other ones but talk with your docs they will decide if the benefit to you out weighs the risk to the baby.

Actually pulmozyme is a cat b drug, which is very safe. Almost no drugs are in a cat a, (surprisingly coffee is considered cat b) so that helps to put safety into perspective since most of us must take some drugs.

I inhaled Tobi during my pregnancy. I was on it when I conceived I believe and then off a month in which time I found out I was pregnant we waited until 2nd tri to start it again and then did it approx. every 6 wks. there out. I did get a peak taken (blood sample right after inhaling) to see what the level of the drug was in my blood. It was lower than the amount they expect to enter the blood during normal use and have found no problems at that level. I was comfortable knowing I was below that level. I wanted to do this because I dealt with hemoptysis and wondered if more was seeping into my bloodstream due to the hemoptysis.

My son has had no problems. Candice, semperfiona (sp?) injected Tobra during her pregnancy. I haven't heard her son has had problems either. Perhaps the deaf fetus' were when we used to inject much higher doses of Tobra. I believe she and I did the typical dose of ~500mg (exact dose based on weight) every 24 hrs.
