Need help with airway clearance...

Our son is 3 and he is on albuterol/pulmicort, pulmozyme, Tobi, azithromycin and The vest (model 105 2x daily for 20 min. settings are 14hz, 4, 20min).

He sounds pretty good most of the time, but has a little gurgle especially at night and in the morning. He doesn't cough very often. If you try to make him force a cough, he tries but nothing real productive. He doesn't even cough on the vest..We also try manual chest pt and some with a flimm fighter g5 too.

What can we do to help get him to cough? What are some other clearance techniques we might try? any other nebs that we are not doing that we should ask about? HTS?

See if he can/will do this:

Have him breathe in a small amount of air, while holding that, breathe in another small amount & repeat until he thinks no more air can fit into his lungs.....then trying huffing it out.

I am not sure how to explain huffing to 3 year old. I guess if you can get him to huff is a big deal....the breathing is just an extra thing that we were taught.

It helps with building pressure behind the mucous.

Varying the settings on the vest might help too- staying at 14hz for the entire 20min might not be giving him the most benefit he can be getting from the vest. I'd talk to his resp. therapist about this. The use of different frequencies and/or pressures can help in clearing both the small and large airways. (I'd discuss it with the resp therapist or dr. before making random selections of course.)

That breathing technique Mel mentioned has helped Emily get out some good productive coughs as well.
