Some of you may recall that I went to the doctor about my never-ending fatigue and pain (plus the other 15 or symptoms I have on almost a daily basis). I got my letter back today. The doc stated that everything was normal, but on the report there are two things that are listed as abnormal and one that is listed as low and of course I have no clue what these are. The first is Leukocytes Esterase, my level was 1+, abnormal. Then WBC 3-5 abnormal. Then MPV 7.8 low. The first two things are on the urinalysis and the last one was on the CBC.

She gave me a referal to a rheumatologist as well. Any help here is appreciated. I also email a friend of the family who is a doctor, but he can take a while to reply.

WBC should be part of your CBC not your urine analysis. WBC stands for white blood cells, which will be increased if you are fighting an infection. It can occasionally indicate other problems as well such as certain cancers or immune system problems, however your ratings over there are so different to ours, i'm unable to tell how elevated they are. If your doc was unconcerned and told you you are normal, then i would suspect the elevation is very slight and therefore nothing to be concerned about.
I think your MPV may be the same as our MCV over here, which is just a measurement of the size of your red blood cells. My guess is you are slightly aneamic.
Your referral to a rheumotologist will be maybe because he suspects rheumatoid arthritis???

I actually heard back from my doc friend and he said that I also might be slightly anemic. He said that they would test the WBC in your urine to look for a urinary tract infection.

The rheumatology referal is for the possibility of arthritis or fibromyalsia. My doc friend said that my symptoms sound like fibromyalsia and if everything else gets ruled out that is the next step.

A positive leukocyte esterase test results from the presence of white blood cells either as whole cells or as lysed cells. Pyuria can be detected even if the urine sample contains damaged or lysed WBC's. The 1+ value indicates that you more than likely have a UTI, or that you've spilling some protein into your urine.

One of the things the lab did with your urine was put it in a centrifuge tube, spin the heck out of it, then collect a portion of it from the top of the tube and look at it under a microscope. The 3-5 means that when they looked at a sample of your urine under a microscope, they observed 3-5 squamous cells, or other noticeable material, in this case WBCs, in the viewing field of the microscope. Any time you see more than 2 WBC's in a high power field (your viewing field on the microscope slide), you can assume you have a UTI.

MPV = mean platelet volume. Basically, the volume and size of your platelets. The normal range that can be detected by CBC lab stuff is 7.8-11. If your bone marrow is doing its job, it's making new platelets all the time. If your MPV is on the low side, that just means your body isn't making a whole lot of robust, strong, platelets right now. Your body is probably just directing its resources to dealing with an infection. (Again, the UTI) and any other infection that CFers like to get <img src="i/expressions/face-icon-small-smile.gif" border="0">

Hope that helps!
