HTS and pulmo can sometimes cause irritation if done together, so some people recommend doing them at different treatments. Most people use it once a day though, so I'm not sure if you could squeeze the HTS in midday or not. My treatment schedule is:

Morning:
Xopenex
Vest with HTS
Azli or TOBI

Mid-Day/Evening:
Xopenex (if needed)
Vest or IPV
Pulmozyme
Azli if on month

Night:
Xopenex
Vest with HTS
Azli or TOBI

When I did pulmicort and advair I did those last.

Inhaled antibiotics should be done after your therapy (Vest, CPT, etc) so that you can treat what remains in your lungs, not what you get out. I was taught to always do Pulmozyme in a separate aerosol. As for HTS, it can constrict the airways, so it's recommended you take a bronchodilator ahead of time. If you look at the studies done by the Aussies, a bronchodilator was given 15-60 minutes before the HTS was started. So if you're nebbing everything consecutively during Vest therapy, you probably want to do your Albuterol/Xopenex/etc before the HTS.
