Proper order in which to do inhaled meds?

Ok, I know this has been asked a million times before, but I'm having a hard time finding a thread for it on here. Can anyone tell me the proper order in which to do inhaled meds? I'm currently on Salbutamol (which is like Albutarol), Symbicort, TOBI, and am now starting Pulmozyme. When factoring in physio as well, what is the proper order in which to do things? Because I don't want to be doing the wrong order, and end up coughing stuff up that should stay in my lungs, i.e. doing pulmozyme after physio, and finding out I should do it before physio.

Thanks so much!

I typically do the treatments and meds in the following order:

Flutter ( get all mucus out that I can)
Albuterol
Vest
Pulmozyme
Tobi
Then cough out if I can anymore stuff

Hope that helps.

Ry's general order was:

Albuterol
Pulmozyme
CPT/vest, usually CPT
antibiotic of the week

Sometimes another round of CPT, depending on how he felt.

I don't know if this is how it is recommended or not but this is how I do my treatments.

Albuterol
Saline
Pulmozyme
... Vest therapy.

When I am on TOBI

Albuterol
Saline
Pulmozyme
TOBI
Vest

Normally the albuterol and saline make me cough without doing the vest. I know it is supposed to make you cough, but the Pulmozyme normally helps me to "calm down" and decreases the coughing a bit so then I am able to do the TOBI. The TOBI also makes me cough some (mainly because it is so strong - it only lasts for the first few breaths). Then I wait a few minutes and do the vest.

Lindsey

My method of thinking is to do the Saline and Pulmozyme before the TOBI because they are supposed to encourage coughing. You want the TOBI to kind of get down in your lungs good to get to the sputum, so I do that after I have done the nebs which increase my cough (once I have stopped coughing). I know it seems to contradict that by me following up with my vest - but that is why I wait a little bit before starting the vest after my treatments.

Hey Liz,

You posted info about the sequence of inhaled meds from the CFF in this thread a while back...

Hehe...thanks Claudette! I couldn't for the life of me find that thread. Thanks for the help guys! One other thing - for those who suffer from hemoptysis, do you find that taking pulmozyme makes it worse? Or do you notice a difference?

The how, when and why of nebulized med

This is the description and order as I posted it on my blog. I also talk a little about how long it takes me to complete the whole process. Go ahead and bookmark it if you like so that you don't have to search through tons of threads in the future

Some stuff does make me bleed. I'm not on Pulmozyme for that very reason--it was lysing healthy tissue of my lungs and not the PA (which wasn't present at the time). I've heard of others who bleed a little bit at the beginning of a Tobi cycle. My theory is that after a month off, the gunk that lines our lungs is given sort of a "jump start" with the strong medication. On a microscale, it's a pretty violent jolt for the lungs, so they bleed a bit in response.

Hope that helps answer your questions a bit and I hope you're feeling well today.
