Pseudomonas - protocol for follow up culture

I don't know what the actual protocol is either. This is very interesting. Austin has cultured pseudo in the past. And was placed on cipro and tobi. about 6 months ago he cultured pseudo again and the doc did the cipro and tobi again but this time Austin is doing the tobi every other month indefinetly.

<br />He only goes to clinic every 3 months also. On occassion when the doc wanted to make sure things were going well we would go back in two months. But as far as cultures go they just do them every three months at his normal clinic days. He is also doing the Tiger II Study and they do sputum cultures for that also so if something would show up i am sure they will let us know.

I was told that this is b/c of insurance, they would not cover more often.

I agree. When my dd cultured PA she had no symptoms, maybe a tiny cough 3 times a day. Ever since I am so afraid to miss something, I culture her more ofen.

Also my clinic told me that we should culutre my dd whenever there is a cough or a runny nose so that we get the most relevant antibiotics if needed.

My son cultured PA at the end of may...we started tobi/cipro for 28 days...whole month of June...I was told to get him cultured again about 2 weeks after he stopped taking the meds. We went to the CF Solutions pharmacy to get it done by the nurse there. It came back clean. A week later was our CF clinic appt and they did another culture. It also came back clean. Ive never had a problem with getting cultures done. Everytime he was sick in the winter and we seen his regular doctor she would culture him and put him on antibiotics and wait for the test results to make sure she didnt need to change him to a different antibiotics. Even at an ER visit when they were putting him on amoxicillin for an ear infection I request he be cultured just incase something was down in there and I didnt want it being covered up with the medicine. A week later when we went for a check up at his regular doctor she was so proud of me for rembering that the med could cover it up and for getting that ER doc to do that culture. They looked at me crazy when I requested it <img src="i/expressions/face-icon-small-smile.gif" border="0"> but I got them to do it!

I would like to avoid the every other month indefinitely path, if at all possible, which is why I would think it would be helpful to culture a month after finishing Tobi and get started on it again if PA is still lurking in there. This was the second time my daughter cultured PA - first time was almost 5 years ago though - she did Tobi/Cipro then too, only one month - and it seemed to do the trick since she didn't culture it again til now.

I am certainly glad to know that I am not the only CF parent with questions about this! : ) Too bad we can't keep some of those throat swabs at home and have a do-it-yourself kit to test every month ourselves!

Hi there

We are in England and our protocol is as follows.

PA culture.

3 weeks of cipro and I think(memory going) 1 month of colomycin nebs.
Then re-swab

PA culture again

3 months of colomycin nebs and cipro.

re-swab

PA culture

Possible iv's and continue with colomycin.

I think that was the order of stuff, I may have got the time frame of some of the drugs a little wrong as i'm going back a few years now.

Regarding throat swabs, we can do them at any time, our clinics are every 2 months or sooner if needed but we also have throat swabs and forms at home so if we want to test, we just do it and drop it off at the lab. Just have to phone the CF nurse to ask them to check on the results.

You'd think there would be a blanket guideline across the board by now.

The vest isn't looked on as being great over here. If we want one, we have to sort it off our own back. Probably a cost issue, I really want to try one for Sophie.
