questionaire before transplant.....

did any of you have to fill out a questionaire right before surgery??? if so, what type of questions does it have on it?? is it insurance stuff or medical stuff?? just wonderign thanks..

I haven't had the transplant, but did have a dry run. Before they sent me down to the OR I signed a bunch of waivers basically saying I acknowledged it's a risky surgery and I may die, and that they explained to me the risks. They asked me a bunch of medical questions (what meds you're on etc.) when I first got there, but I don't really remember any paperwork for that. I also remember signing something when I got to the OR waiting room saying that I was informed the donor was from the extended risk pool. He had briefly used marijuana and cocaine 30 years prior, so they were legally obligated to tell me.

No, I didn't have any questionaire, but they do ask you how you feel, have you had a fever recently. I also signed consents for the surgery itself, for the high risk donor, and for anaesthesia.
