we use the fish mask as well. I just took off the string and hold it. Ask your clinic they just gave us two to start with and told us to let them know if we need more.

ask your Doc. to write a script for more than you need so that you will have extra next time. It's always nice to have an extra in case one falls on the ground or you need to do an extra treatment. If your team has a respiratory therapist, that is the person to ask for a slightly larger size.
Also, you could probably get your doctor or CF nurse to write a letter to your insurance explaining how you need a different mask size.

I agree with others... try asking the CF Clinic. Ours has always been helpful.
