My son w/cf (age 46) has used Spiriva for close to a year now. As far as I know, he continues to use it, but (1) Does not think it has helped much, if at all. (2) Has had NO side affects at all. Me, is another story. I have bad COPD and was on a nebulizer, with 2 meds, 4 times a day. I started Spiriva as soon as it came on the market here in the states and have been on it since. My lungs/breathing/general health have all improved greatly! Also, I have not use the nebulizer since the day I started. Two puffs a day versus minimum of 30 minutes, 4 times a day has given me at least 2 hours more a day to do as I want. Plus, I've had no side affects! I like it!!!! However, as we all know, every one reacts differently to any med. I know people who rave about Advere (?). I tried it and within 36 hours I thought I was a goner for sure. Had to stop it immediately and won't be going back to try it again. My son w/cf is also currently on Advere. He seems to think it helps, a little. I personally believe we should try anything new that comes on the market (as long it doesn't harm us) and see if maybe this is the one that just might help me/you. Good luck. Bill
