You may not be able to fully get 'proportional' as you are hoping.

But yes, you are correct that the only way you would build your arms/legs would be exercise to build the muscles.j We don't store noticeable amounts of fat in our limbs so the size is defined by bone & muscle.

I will clarify that changing enzymes *might* lead to overall weight gain if you are small, but it is going to be in your torso not your arms/legs

Tom has obviously never seen anyone with fat legs. After five years as a lifeguard on an Atlantic Ocean beach and not wanting to argue with the good Doctor, I can tell you that lots of people "store" fat in there legs.

I don't have CF, but I'm convinced that I'll never be able to gain weight in my arms! They're super long and slender! My butt and legs have no problem gaining weight - my arms don't seem to have the fat cells to store the fat! Just the way it is with my body. Used to be self-conscious about them...but it is what it is! I've had three babies - my arms never got any bigger with weight gain associated with pregnancy and I surely did gain a lot of weight! May just be your build...not malabsorption per se. Guess I didn't inherit the big arms on my Italian side of the family...

Bill,
I think what Tom is saying is people with CF. I'm sure he has seen people with fat legs.

As for fat storage I believe hormones play a significant part in that, where testosteron (mostly male) generally makes fat to go to belly back and upper body and estrogen (mostly female) generally makes fat go to thighs butt arms and so on. (notice the generallies)
However above some level fat is not very useful weight so musclemass is what you want but CF tends to make it harder for our bodies to keep on musclemass, as well as often being less active when we feel sick etc. As well as sometimes having an enlarged midsection and ribcage.. So that sucks.
But weightlifting helps a lot.
