TOBI for 2 weeks

My dr. recommended doing 2-weeks of inhaled TOBI as a preventive before my husband and I do our first IUI.

Well, due to delays in getting the TOBI in the mail, I am now 2 days before IUI (pending ultrasound/ follicles). My DH and I are wondering if we shouldn't go ahead w/ the TOBI since I would start today, and be on it almost the whole 2ww post iui. This will be our first iui, and I am really quite healthy (PFTS > 100%).

I plan to call the clinic today. I emailed my dr. last night, but she missed that portion of my email b/c she didn't respond to it (only another question was answered).

Hoping someone can give their opinion or experience.
Thanks so much - -

(I am nervous/excited about the whole iui process, trying not to get too excited since it can take multiple cycles. We are ttc #1 and are into month 18 now).

Hi Christina!

My doctors feel like there is nothing wrong with being on inhaled TOBI at any point during a pregnancy. It's not absorbed into the blood stream so it really shouldn't have any effect. I definitely wouldn't worry about being on it. I've been on and off it throughout my entire IUI process.

Hope this helps and good luck to you!

hi christina,
i wish you the best during this exciting time!

i'm in a similar boat with you, i hope to start ttc soon (this would be going into a 2nd preg for me, so i've been through all this before) and am beginning the med switches on a few things.
from my research it seems like most docs lean towards using tobi during preg, if necessary. its the risk/benefit decision to make.

but the jury is still out on tobi's 100% safety. tobramycin itself is a category d drug for preg-and therefore not recommended for preg. but-since tobi is inhaled, the amount that would cross the placenta is def much less, if at all. basically, they just don't know alot about tobi and preg bc there hasn't been alot of testing.
my doc is very-sometimes overly-cautious about drugs during preg and bf. she does not want me to be on tobi during preg, bf or ttc-she is adament about it. the other part of her philosophy i think is that i should be healthy enough for a period of time before preg, to not need tobi or abx.
we work together a year before ttc to plan it all out properly and keep me as strong as possible.
of course, you go into preg with the best intentions and planning, yet things can still come up.
a healthy mommy is a healthy baby. so, if the need for abx came up-i'd use them directed by my doc.

i can't tell you what to do, and i'd never tell a patient not to listen to their doc.
that being said, i can only share my experience (as a healthy mom, with no preg complications)-and i can only share what i would do in your shoes.
i wouldn't take the tobi. for a few reasons-one, the treatment duration for tobi is 1 month. so doing 2 weeks might not do anything. if its not broke, don't fix it.
2nd, if you feel great-not ok, btw, great-why do you need abx? your pfts are fantastic-the same as a non cfer.
3rd-going back to tobi, its still an abx, and i try to reserve my abx for when i truly need them. we do have resistance issues.
now that being said, do i think doing 2 weeks of tobi will harm your baby or chances of conceiving?-no. i think everything will be fine.
its a personal choice. theres no one size fits all routine when it comes to cf and preg.
ask yourself-what will give you the best peace of mind?
for me-personally-its keeping my body off whatever drugs i can while preg, and being 100% compliant with the ones than i will stay on.

above enjoy this special time and don't stress-that will help you conceive too!

I Kind of echo what Christian said. If you aren't actually sick or feeling less than 100% then if I were in your shoes I wouldn't do it. Even though it is inhaled some of it does get into our blood stream, though much less than through IV (I vividly remember reading about a little boy who was getting ottotoxic from inhaled TOBI and when they checked his blood levels they were very high - very very rare but does happen). I doubt it would harm your embryo should your IUI work but if you don't really need it then why take the even minuscule risk (IMO!).

As far as bf goes (since christian mentioned it) Tobramycin in any form is actually perfectly OK to take while breastfeeding because tobra is not absorbed orally - hence why we don't have a tobra pill. So even if you had massive amounts in your milk (which you wouldn't) baby would not actually absorb any of it into his/her system. All that info is per Dr. Hale.

Best wishes on your IUI!!!!
