Sean's been put on Tobi for a month. It's been years since his last round of Tobi so I need a refresher...

What neb should he/shouldn't he use? I seem to remember several discussions on here about which neb cups are good to use with Tobi and which aren't.

The nurse told me to make sure he does it last and not with his vest. However, after HS Sean coughs a lot so I'm wondering is doing Tobi after HS is a good idea?

I think the reason they want us to do it last is so it can get into our lungs better after we've coughed a bunch of stuff out. However, I always do it first because its just so harsh.

If your son can't handle doing it last, then I would suggest letting him do it first. There's no sense making him cough until he thows up.

I think the neb cups which are good to use are the tall one from PARI. The last time I was in the hospital, though, they just used regular neb cups. I think the main thing is how much time it takes for the TOBI to be finished. I think they say it shouldn't take more than thirty minutes. If it takes longer than that, the TOBI isn't as effective, or something.

Yep, Tobi last so you don't cough it out doing the HS, vest, etc. It needs to stay in your lungs to work. It also can get deeper into the lungs after he's coughed all that gunk up with the HS. You could always wait for a few minutes between the HS and the Tobi, maybe give him a drink, so it's not as bad. Personally, I do my Xopenex, HS, vest (start it at the end of my HS or maybe half way through HS I'm not coughing a bunch at that point), Pulmozyme and then my Tobi last- it gives me a break between HS and Tobi.

Oh, and it's the Pari LC + neb with the Pari compressor (there are a couple of other compressors, but I don't know for sure what they are anymore).
