Tobi SOB

OK....I need advise. I have been fighting a cold for about a week. You know...the crappy kind that starts out with a dry cough and then blah. I was not on anything inhaled or other. I have not taken Tobi for about 6 months. I was cycling it until I started AZLI and then decided to take Tobi out of the mix because it just has caused me problems here and there.

Well I tried to fight this cold on my own; but, decided today to have my PCP call in Levaquin 750mg and start Tobi until I get my AZLI next week. I DO NOT like doing mono-therapy....so I wanted to do both. The Tobi really has kicked my butt today and I am really feeling SOB. YES...it is the Tobi cuz I was fine until I did it this am.

My question: Should I skip the Tobi tonight and maybe all together until I start the Aztreanam next week and call the Levaquin good? I am just so concerned about resistance with just one abx. Also, since I did one dose of Tobi....I am concerned about that being in my system and then just coming to a halt.

I know you all will tell me to ask my docs; but, as you know a lot of these decisions they simply leave to us. Any thoughts would sure be appreciated.

I feel that I should insert some sort of medical advice disclaimer here! lol

I'm probably not a good person to ask, but if I were terribly SOB with the TOBI, I would probably hold off until I start the AZLI.

I'm only saying this because I used to take TOBI and it made me repressively SOB and I decided that the quality of life just was not worth any benefits the TOBI may have offered.

Let us know how you're doing & I hope you're feeling better in no time!

Hi Skye,

You are right. Ask your doctor. I guess I just don't understand when CF doctors are having the patients make decisions. I don't know what you should be taking because I don't take antibiotics when I have a cold -- then again I take enough home remedies that I don't really get real colds anymore -- I know that doesn't help you much.

But I will say this, when I start Tobi and then want to stop, my doctor won't let me once it's started. So that's why I would ask the doctor since you only took it once. Would stopping the Tobi after one use affect resistance?

Tobi causes SOB because it causes an asthmatic response -- it is irritating. What I do is this -- I do my bronchodilator (Foradil or Brovana if I am really sick) first then I do all my treatments then by the time I get to Tobi it is way later. I take 1 or 2 puffs of Xopnonex HFA to open the lungs back up since some treatments like HS can be bit harsh when you are sick. Then I do Tobi. If you want to do this, you must ask your doctor, because you don't want to take too much of a bronchodilator. And he can or least should advise you on how to avoid the SOB.

As far as all the other stuff, I can't recommend anything when it comes to antibiotics, but if you ever want to know how to avoid colds, or what I do when it is coming on, or control the asthma component part of CF -- that's something I know a little bit about.

I hope everything works out well for you without you getting sick. Fresh Ginger tea with honey sooths a cough really well.

I understand your worry about resistance, as I worry about it all the time. That said, it probably doesn't matter much which route you choose to get to your AZLI next week. You're probably ok anyway you slice it. A few days on Levaquin solo doesn't sound like the end of the world, particularly if you're going to be polishing off the course with the AZLI. If it were me tho, I'd try the tobi one more day to see if things improved. If they didn't, then I'd seriously consider bailing. I hate making these decisions! My CF doc doesnt' seem to know a lot about resistance. Personally, I'd like to speak to an infectious disease doctor about this kind of stuff.
