Tobramycin resistant pseudo

My most recent sputum culture showed two strains of abundant pseudomonas resistant to Tobramycin. I've been doing every other month TOBI and am going to switch to the podhaler next month. I asked my clinic about the resistance, but they weren't too concerned. Doing my own research I found the way labs interpret resistance may not be terribly valid with CF med dosing. Any thoughts?

Yes - there are different categorizations of "resistant." I believe I am technically resistant to Colistin and Cayston, but they both work. You can be sensitive to an antibiotic, meaning that it should be totally killed; resistant just means it isn't all killed. I think there's some sort of "total resistant" category and you're probably not there. Plus even if you're resistant right now you could become sensitive later. I was on TOBI for a decade and I became so resistant it seemed to make me sick - I got sick one 3 or 4 TOBI cycles in a row, during times I am normally healthy - so I went off of it, but I would be willing to try it again after some time passes. It's definitely scary to read the culture results and see almost everybody antibiotic have "resistant" next to it but it's not as bad as it seems!

Hey Red,

My doc has basically said that he finds culture results/sensitivities to be useless. He says that in a petri dish it might show someone is resistant, but when given to them in real life, it works. Also, sometimes it shows someone is resistant to several antibiotics, but when used together the combo is very effective.

I'm guessing since your doc isn't too concerned that they prob have about the same opinions as my doc.
