When do you call the CF clinic?

Quick intro: I have cf and have a son with 2 mutations (a-typical cf) who is 6 months old. He does not have a cf diagnosis because he passed the sweat & newborn tests which reflect functioning CFTR and pancreatic sufficiency. Also, he has his vas deferens. Further testing showed that he was PS also. He shows no symptoms and is 19 lbs. at 6 months.

We have prepared both emotionally and physically (getting the gear in house) in advance of any symptoms. Since finding out about his cf he has had a head cold that went South and produced a loose cough for a couple days. We did cpt 2x/day and albuterol neb before the cpt. It was probably overkill but I figure it's better to be safe than sorry. The cf center told us as long as he was eating, good disposition, cough not disrupting sleep, no wheezing and no fever to contact the peds that they would consider this a non-cf, run-of-the-mill type cold. They said same goes for ear infections. So that cold was 12/26. Well Friday he came down with another cold. Again lots of mucus from the nose but totally clear in color (just like last time). This time though the cough sounds a little deeper and stickier...more difficult for him to clear. It is very infrequent. Like once an hour and he isn't coughing in his sleep. He had a fever Friday and Sat. but none today. We've been doing albuterol and cpt 3x/day per direction from the peds. Sometimes I can feel mucus vibrating in his left lobes. Would you call this wheezing even though there's no noise from his mouth on the exhale?

My question for you is this....

When do you involve the pediatrician?
When do you involve the cf clinic?
Are these guidelines you've decided or have they asked you to follow this protocol?
In your non-cf kids did they get colds that would produce a cough?
Could you tell me about a typical cold for your non-cf child under age 1? What did it entail?
When you suction mucus from the nose is it stringy (meaning a high tenacity)?
How many colds did your cfer get in the first year of life?
How many colds did your non-cfer get in the first year of life?

IMO call the CF clinic. He may just need a little extra help getting rid of whatever bug he's got.

My experience with the local clinic is they think it's normal for him to cough "Cfers cough, it's what they do". We were told by his primary CF doctor in the City to "learn his cough" and wants us to call when DS coughs and usually prescribes an antibiotic. I didn't think his last cough was a big deal, which was more of a sporadic, throaty, swallowing wrong cough -- but at his last clinic appointment prescribed Cipro for a months because he didn't like the way it sounded when he had ds cough. He's an infectious disease doctor, so tends to be very proactive.

When DS was 3 months old he had bronchitis and was culturing an enterbacter choclae (sp) bug he brought home from the nicu. He'd been coughing for several weeks, throwing up his formula. He got put on septra and tobi. We got a lecture from the doctor for not calling him. We'd been told at his 2 month well child visit his cough was normal. We were already doing CPT 4 times a day with duo neb while I was on maternity leave, so we didn't do anything differently there.

Oh and once DS started daycare and I noticed he was wheezing. His doctor told us to "beat the cough out of him" we went back to the 4 Cpt/neb treatments a day.

DS seemed to have sinus issues -- in fact it seemed like his first winter he had a constant stuffy nose. I didn't EVER see his nose run until he was 2 1/2 and started pulmozyme. We constantly were using saline and suctioning his nose. It was nasty, rubbery stuff. Didn't get many colds -- he had 3 ear infections his first year. October, January & May. He did get a little wheezy once or twice. No fever, no cough.

We don't have any other children. I have noticed one of his cousins ALWAYS has a nasty boogery nose. This past Xmas was the first time I've ever seen her without one. She just turned two.

First of all, this is a hard one to deal with..who do I call? What kind of sick is this? Then, in your case you have the added issues of YOUR cf so naturally you are going to treat his cf like symptoms like yours, and then throw the "atypical" stuff in there...well, no wonder you are wondering!

My non-cfer definately has colds that come with a cough. Honestly, there are times when she coughs more than the cfer. The first year I think she had a runny nose more often than not, usually clear, sometimes cloudy and always stringy in the bulb syringe. It grossed me out as a new mom to use that thing. Whenever I pulled it away from her n ose the stuff would pull out like a spider web. Anyway, in the first year I'd day she had 4-5 colds. Minor ones, just a week or so. She usually had a cough, runny or stuffy nose, decreased a ppetite. She went to daycare, Gymboree...also had a good number of ear infections and had nebulizer treatments at the peds office 2-3x for that "rattle" you described. By 18 - 24 mnonths that stopped. Now she gets 2-3 colds a year, I'd say...usually when I get mine, with t he weather change or about 3 weeks after school starts.

The cfer had 2 colds the first year. I re member because I panicked both times. I have a LOT of faith in your pediatrician who actually did her residency inn pulmonology and knows her cf stuff. So, I call her first for these kinds of things. She is great about saying, call the clinic, and she and our pulmo have been known to e-mail or call back and forth to confer.

I say call the pediatrician. To me, it does sound like a run-of-the-mill cold. But, its so hard to figure that out, the first time around, especially. YOu can always call the c f clinic later if things get worse. Go ahead and neb the abuterol, maybe 1x/daily and see if that helps.

I hope he's feeling better soon so you can feel better about it, too.

Oh, and 19 lbs at 6 months???? WOW! Emily was 19 lbs at 18 months when she got her feeding tube. My non cfer was 19 pounds at about 16 months. I'll bet he's an adorable. squishable guy! Give him a hug for me!

Oh, and 19 lbs at 6 months???? WOW! Emily was 19 lbs at 18 months when she got her feeding tube. My non cfer was 19 pounds at about 16 months. I'll bet he's an adorable. squishable guy! Give him a hug for me!

First of all, this is a hard one to deal with..who do I call? What kind of sick is this? Then, in your case you have the added issues of YOUR cf so naturally you are going to treat his cf like symptoms like yours, and then throw the "atypical" stuff in there...well, no wonder you are wondering!

My non-cfer definately has colds that come with a cough. Honestly, there are times when she coughs more than the cfer. The first year I think she had a runny nose more often than not, usually clear, sometimes cloudy and always stringy in the bulb syringe. It grossed me out as a new mom to use that thing. Whenever I pulled it away from her n ose the stuff would pull out like a spider web. Anyway, in the first year I'd day she had 4-5 colds. Minor ones, just a week or so. She usually had a cough, runny or stuffy nose, decreased a ppetite. She went to daycare, Gymboree...also had a good number of ear infections and had nebulizer treatments at the peds office 2-3x for that "rattle" you described. By 18 - 24 mnonths that stopped. Now she gets 2-3 colds a year, I'd say...usually when I get mine, with t he weather change or about 3 weeks after school starts.

The cfer had 2 colds the first year. I re member because I panicked both times. I have a LOT of faith in your pediatrician who actually did her residency inn pulmonology and knows her cf stuff. So, I call her first for these kinds of things. She is great about saying, call the clinic, and she and our pulmo have been known to e-mail or call back and forth to confer.

I say call the pediatrician. To me, it does sound like a run-of-the-mill cold. But, its so hard to figure that out, the first time around, especially. YOu can always call the c f clinic later if things get worse. Go ahead and neb the abuterol, maybe 1x/daily and see if that helps.

I hope he's feeling better soon so you can feel better about it, too.

We made sure in Michigan that we had a pediatrician with CF experience as well as the willingness to keep updated on CF news. Once we moved to North Carolina, I had our CF doc refer us to a pediatrician in Greensboro who knows CF. In fact, they worked together at one point. So, there is always communication between the two of them and if my ped is stumped, he does not hesitate to call our CF center.
