How long does your child do the vest?

We have Emily use the Minnesota protocol (developed by Dr. Warwick) where you use higher pressures with lower frequencies/hz, and then gradually decrease the pressure as the frequency goes up. This helps target both the small and large airways in a way that was shown most effective in his studies.

Thanks for all the replies everyone. Right now DS does 10 mins at a lower hertz and then increases to a higher hertz the last 10 mins with the pressure remaining the same the whole time. PT in Chapel Hill said changing the hertz from lower to higher breaks up the mucus better. I don't remember her saying anything about changing the pressure. I will definitely have to ask about that at our next appt in Jan - thanks for the insight!!

Emily is 5 and follows the "Minnesota protocol" as perscribed by our clinic RT. We started out (when she was tiny) at 20 min 2x/day, and even then there was some variation in the pressure/HZ settings. We've changed it up over the years (WOW, it really is years), and occassionaly had to adjust to her current likes and dislikes. She tends to like it "fast and hard."

I don't have our current protocol setting written down anywhere, so I can't be too specific right now about pressures and speeds, but, each session lasts for 24 minutes, total. Each phase has it's own settings, starting slow and low pressure, then going up and coming back down.

We've actually tacked a minute or two on to some segments so that she vests for closer to 30 minutes. This matches her neb time, AND the length of an episode of Phineas and Ferb (or whatever strikes her fancy).

We also stop between segments to huff-cough now. This has been so beneficial for Emily. She gets stuff up while vesting, and can also huff junk out when she needs to (Like during a sputum sample to avoid the dreaded swab!!!)

I would reccommend asking your RT about Minnesota protocol. Especially if your vest can change settings automatically.

Minnesota protocol in a nutshell calls for using- if you have a HillRom vest:
6, 8, 9, 18, 19 and 20hz for 5min each. You start at the highest pressure your child can tolerate (turn the vest on to inflate it and gradually increase pressure until you are at the max your child can tolerate comfortably; this is considered 'starting pressure') with the lowest frequencies, and gradually reduce as the frequencies increase. By the time you are done, the pressure is several lower than where you started. In our case, Emily cannot tolerate 20hz, so we do 6, 8, 9, 18 and 19hz for 6min each. She starts at a pressure of 8 and gradually drops.

If you have an InCourage system it's recommended to use the pre-programmed setting that cycles through the recommended speeds and pressures, which will give you the ideal mix to target all airways in the way that his studies show are most effective for that type of vest.

Aidan went to Minnesota and we use their protocol. QuickStart on the Respirtech twice a day for 30 minutes. At age 6, his lungs are awesome!

I am planning to start my asymptomatic 2 year old on the vest come January and I find the idea of 20-30 min 2x a day very very daunting. Would it be completely bizarre of us to start with 15 min 2x a day and ramp up? She has no respiratory symptoms as of yet, is pancreatic sufficient, has never been sick or cultured anything, is big and healthy and vigorous, etc. etc. so it's not like there's a big backlog of goop in there that we have to break up the first month we start with the vest I don't think... or is that a wrong-headed way to think about it?

They wouldn't even be giving her a vest were I not sort of agitating at them for one. I feel weird telling doctors to treat her?

She's at an age where it is a serious challenge to put on her shoes, I am going to need a prescription for Valium (for me!) in order to start her on the vest

We started with 20 minutes at 8/10/12 with a pressure of 5. That was just what the Hill Rom Rep set us up with. We've since increased the pressure and time -- similar to the U of MN protocol, but ds gets stressed at 18 hz plus. I so wish we had a vest that was programmable, but we were prescribed Hill Rom and their programmable one came out a few month after we got ours. Oh well. I'm in the room with him anyway, so I change the settings and remind him to cough.

He thought it was pretty fun at first as it made his voice sound funny so he's sing. He was 3 at the time. He did give us trouble when he couldn't hear the tv or felt tied down, but with the discovery of Lego games on the Xbox and only playing during vest times, he's start ASKING to do the vest.
