Dr. Advised Hypertonic THEN Pulmozyme

I went to my Dr. appt. today, and she did tell me to switch the HS and pulmozyme order...HS first, then pulmozyme. She said that's how they presented it at a conference she went to, and that that's what the CF Foundation is now recommending. I still haven't found the study or recommendation on their website though. *sigh*

Good grief. Maybe I'll just do it pulmozyme the HS in the morning, then HS then Pulmozyme at night, or vice versa LOL so I'm covered!<img src="i/expressions/face-icon-small-confused.gif" border="0"> LOL! But really, *sigh* I'll ask at my next appointment... Anybody find the study yet? <img src="i/expressions/face-icon-small-smile.gif" border="0">

I doubt you will find any studies on this that show any significant order benefit. I think if you are being compliant doing both HS and Pulmo then you have 98% of the benefit. Maybe there is something like a 2% benefit of doing one before the other, but I believe that will just be theory of what makes sense and not be able to be proved significantly in a study.

You can always experiment and see what works better for you. Whatever gets the most junk up for the individual is all that matters. Switch the order for a week and see how it works.

My doctor has had me doing Hypertonic before Pulmozyme ever since I added HS to my routine, which was probably a year and a half ago.

I never asked the reason for the order, but I've always done HS before Pulmozyme and it seems to work swell!!

I'd be interested to find a study supporting that!

She indicated that there are actually a few studies (some ongoing and some that are completed) evaluating the sequence of inhaled meds. Beyond the general order (i.e. Bronchodilator, Mucolytics, Inhaled antibiotic, Long acting bronchodilator, then Inhaled steroids), she is recommending to patients that are on both HS and Pulmozyme to experiment and see what works best for them.
