faking it - 7% HS

Well I've confirmed what I suspected for some time. We switched Katy to a mouthpiece to do Hypertonic Saline, which she calls the "scratchy nose" medicine, and she can't tolerate it very well at all now that she is actually breathing it in. She was clearly cheating it somehow when nebbing it with the Bubbles mask.

She nebs Albuterol prior to HS. She can take just so much of the HS and then has a dry, irritated cough. She says her throat hurts. She has yet to be able to finish the 4 ml. I thought I would get your opinions as well as call the doc. How long did it take (if at all) your child to adjust to HS? If the 3.5% isn't as harsh, is it still doing any kind of job at all? Any other advice or opinions? <img src="i/expressions/face-icon-small-smile.gif" border="0">

Dana,

Maggie does her HTS(7%) with the mask always. She occasionally uses a mouthpiece, for instance when we are traveling. I like using the mask for the saline to get up her nose/sinuses. When maggie first started the HTS; she coughed for a month, however she also had an upper respiratory infection at that time, too and needed antibiotics as well; then the cough stopped. Maggie usually only coughs with the HTS when she is coming down with a cold. She may not get as much saline in her lungs with the mask but I do see her breathing it in. Maggie also requests a drink after her HTS, makes her thirsty I guess. It is also suggested to wait 15 min after the albuterol before starting the HTS. One of the other reasons I stopped using the mouthpiece with the HTS was because, Maggie would occasionaly DRINK the HTS from the mouthpiece and say ALL DONE! When Maggie first started the HTS , I asked about starting at a lower concentration and our CF doc said no. Does she tolerate the HTS with the mask?

Oh, I saw that you said she calls it the sratchy nose medicine. I do remember Maggie compaining that her nose hurt from it as well. If it makes you feel better; it does not bother her nose anymore. She even got an irritated area on her nose from the salt. And I had to switch to the mouthpiece as well, then she drank the HTS, then back to the mask, now she's fine with it. Someone here suggested putting, Vaseline over the irritated area(not inside her nose) it was more on the outside of her nose. Also, what about doing the Albuterol with a mask, so that gets up her nose, too? Sorry I'm tired hope this is not too confusing!

I have a hard time with the 7% too so I only use the 3% and it still does a great job! Actually, I think it works better b/c I can breathe more of it in and get a more productive cough vs. the 7% I cough almost non stop from irritation so it doesn't seem to get down into my lungs as well b/c I can't take as deep of a breath and makes my cough drier and less productive. I would definately try the 3%!
