Unlike most people, we do not do the bronchodilator. The first time the boys tried HTS, they did have that dry, irritating cough. So I diluted it to half HTS and half sterile saline solution. That completely eliminated the irritating cough. I very slowly added more HTS then sterile saline and we are now giving them the whole amount of HTS without a bronchodilator and no irritating cough.

<br />As a side note, this was never recommended by the docs. The boys are at a point where they are very healthy lungwise and thus I do not want to add medication into their regime unless it proves necessary. We will add the bronchodilator, if it seems there is mucus in their lungs that is not coming out.

Yes, we do albuterol first. We have the option of the inhaler, but we neb it- inhalers don't seem to help her very much. The bronchodilator helps open things up so the other inhaled stuff can get deeper into the small airways. Also, hypertonic saline, beneficial a treatment as it is, it can also cause bronchospasm in some people, which a bronchodilator can help prevent. Emily has a pretty bad asthma/inflammation component to her cf and does fine with hypersal, but I am sure doing the albuterol first is essential, since she has needed it all along.
