I've inhaled gent before. It is in the same class as tobramycin, it's actually more widely used in other parts of the world, as it's more available. It is not the same as TOBI, rather more similar to tobramycin. TOBI is meant for inhalation, tobramycin IV and gentamycin IV are not meant for inhalation. All of us old school CFers used to inhale tobramycin IV long before TOBI was around. Personally, I prefer it, at my old age, me and TOBI don't get a long.
I got the 2mL pre-mixed IV vials and dumped two of them into my Pari LC Plus nebulizer with some xopenex (or albuterol) I used those really tiny bottle openers (like for beer lol) to flip the caps off the tops of the IV vials, instead of dealing with syringes. It's a long treatment and doesn't taste all that great. Some people have a hard time inhaling gent or tobra (IV) because of the preservatives can actually cause bronchial constriction. That's why it's always a good idea to use a bronchodilator beforehand (or concurrently). I've never felt "tight" after using tobra IV ihaled or gent IV inhaled.
Good luck.
