How do you know it is time for a vest?

Not necessarily OT, but have any of you (or your children) coughed more since switching from manual CPT to the vest? We're still doing manual CPT effectively...

Aunt Cob: I definitely think you need to evaluate your doctors recomendation of only using airway clearance when your son is "sick". I'm sure that if you did it on a constant basis, your son would most likely raise his pfts... in the 80's isn't horrible...but it's not good either.

Idenkers: Just because your twins don't have any symptoms yet, doesn't mean they shouldn't be treated from the start. It's important to keep the secretions moving, so bacteria don't harbor in their lungs, and also to get them used to it. Cf is progressive, and they wil NEED the treatments to keep them well in the future. By doing them now, you get them used to it, and perfect your technique. Depending on their size, you might be able to get vests for them. My son started the vest at 1 yr. Since your sons haven't shown any symptoms, it would probably be hard to get an insurance company to approve one sooner. Even if you dont use a vest, manual cpt should be done daily. I'm not a doctor, but our clinic prescribed 2xs for 20-30 min when my son was an infant. now that he has the vest, he does it for 30 min, twice when WELL... 4 times a day when sick...and every 4 hours around the clock when in the hosptial.

If they have not taught you, it's time to ask the respitory therapist to show you how. There are videos online, or books you could look at.

Katie: yes, Emily coughs more with the vest than either cpt or the acapella (although she doesn't have the stamina to do as effective a session w/ acapella as she really should, so that one is hard to judge in effectiveness in comparison to the vest. I like the premise of how it works, to vibrate and loosen secretions from the inside (you can even neb while using it), though I don't see it often mentioned here that children or teenagers use them.)

I am glad we didn't go straight to the vest w/o learning how to do cpt first at the time of her dx (i.e. 'Emily's 7, so don't worry about it, she'll have the vest soon.) It's a vital skill to know no matter what the age- vests break, you may not want to travel with it, etc- and if Emily ever had pneumonia or a problem in one specific area of her chest, I could do extra cpt in that trouble area to help keep it clear even while she continues her regular vest sessions. However, I am SO glad we have it. Our mornings are so busy trying to get 3 kids ready for school and the vest ensures much greater compliance, is allowing her to learn a measure of independence at even a young age, and she's so much more cooperative for it than she was for cpt.
